头皮恶性肿瘤切除后创面局部皮瓣转移联合游离皮片移植术修复11例分析

摘　　要：目的探讨头皮恶性肿瘤切除后形成的创面应用局部皮瓣转移联合游离皮片移植术修复的临床效果。方法回顾分析2017年1月至2022年1月在焦作市人民医院皮肤科治疗并完成随访的11例头皮恶性肿瘤患者资料。患者年龄30~91岁,男4例,女7例,基底细胞癌6例,鳞状细胞癌5例,所有患者均采用局部皮瓣转移联合游离皮片移植术修复头皮恶性肿瘤切除后形成的创面。结果11例患者均一期手术完成,所有转移皮瓣和移植皮片均成活,术后均未出现感染,切口甲级愈合。术后随访6~12个月肿瘤无复发,创面修复良好,患者满意度高。结论局部皮瓣转移联合游离皮片移植术是头皮恶性肿瘤术后创面修复的理想方法,具有手术一期完成、转移皮瓣及游离皮片易于成活、肿瘤复发率低、恢复良好的优点。Objective To investigate the clinical efficacy of combining local flap transfer with free skin grafts for wound repair following resection of malignant scalp tumors.Methods The retrospective analysis was conducted on the data of 1l patients with malignant scalp tumors who received treatment at the Dermatology Department of Jiaozuo City People's Hospital from January 2017 to January 2022 and successfully completed clinical follow-up.The patients ranged in age from 30 to 91 years,comprising of 4 males and 7 females,with a distribution of 6 cases of basal cell carcinomas and 5 cases of squamous cell carcinomas.Reconstruction of the wound resulting from resection of malignant scalp tumors was achieved through local flap transfer combined with free skin grafts in all patients.Results The one-stage surgery was successfully completed in all 1l patients,with a 100%survival rate for the transferred flaps and grafts.No postoperative infections were observed,and the incisions exhibited excellent healing outcomes(grade A),resulting in a high level of patient satisfaction.No tumor recurrence was observed during the 6 to 12-month follow-up period.Conclusions The combination of local flap transfer and free skin graft is an optimal approach for wound repair following resection of malignant scalp tumors,offering the advantages of one-stage procedure,high viability of transferred flaps and free skin grafts,low tumor recurrence rate,and favorable recovery outcomes.
