甲下鲍恩病1例

摘　　要：56岁男性患者,右足第一趾甲下新生物伴渗出1年。皮肤科情况:右足第一趾甲下可见一1cm×1cm×0.5cm大小的黯红色质地稍硬的肿物,甲下及甲周可见黄红色脓性分泌物。皮损组织病理检查:部分表皮缺如,表面较多坏死物,棘层增生肥厚,增生细胞排列紊乱、大小不等,可见异常核分裂像;真皮毛细血管增生扩张,较多淋巴及组织样细胞浸润。诊断:甲下鲍恩病。采用拔甲后手术刮除肿物并联合光动力治疗5次。随访6个月无复发。A 56-year-old male patient presents with a neoplasm located under the first toenail of the right foot,accompanied by persistent oozing for a duration of one year.The dermatological examination showed that a dull red,slightly hard neoplasm measuring approximately 1 cmx1 cmx0.5 cm was observed under the first toenail of the right foot,accompanied by yellowish-red purulent discharge.Pathological findings of skin lesions revealed partial absence of the epidermis,with increased necrotic material on the surface,hyperplasia and hypertrophy of the spinous layer,disorganized arrangement of proliferating cells,unequal size,and abnormal nuclear division images.The dermal capillaries were hyperplastic and dilated,with increased infiltration of lymphocytes and histiocytes.Diagnosis:Bowen's disease under the toenail.Treatment:the patient underwent surgical curettage of the neoplasm following nail extraction,combined with five sessions of photodynamic therapy.The patient was followed up for six months and showed no recurrence after treatment.
