神经鞘瘤1例

摘　　要：45岁女性患者。左股部肿物6年余,缓慢增大,无自觉症状。皮肤科情况:左股部伸侧可触及一2cm×3cm椭圆形黯紫红色皮下结节,质地略硬,境界清楚,无明显压痛。皮损组织病理示:肿瘤团块位于真皮深部及皮下组织,近圆形,有包膜,可见典型的Antoni A区及Antoni B区;Antoni A区由密集的梭形细胞组成,细胞核长,两端细,呈波浪状,部分细胞核排列成栅栏状形成特征性的Verocay小体;Antoni B区可见散在分布的梭形细胞或星形细胞嵌在大量疏松的黏液样基质中,小血管壁可有玻璃样变性。免疫组化染色显示肿瘤团块内梭形细胞S-100、SOX10均弥漫阳性;肿瘤团块周边包膜细胞EMA阳性;肿瘤团块内阿辛兰染色阳性。诊断:神经鞘瘤。手术切除,随访3个月无复发。A 45-year-old female with a nodule on her left thigh for 6 years.Dermatological condition:there was an ovoid dark purple subcutaneous nodule about 2 cmx3 cm in size on the left thigh which was slightly hard texture with a clear boundary and without obvious tenderness.Histopathology showed the tumor was located in the deep dermis and subcutaneous,ovoid and encapsulated,with typical Antoni A and Antoni B areas.Antoni A area was composed of dense spindle cells with tapering,enlongated,rather wavy nuclei;there was prominent nuclear palisading producing the distinctive Verocay bodies.In Antoni B area,scattered and spindled or stellate cells were set in an abundant loose myxoid stroma,and the hyalinezed walls of the small blood vessels were visible.Immunohistochemistry results showed that S-100 and SOXi0 were diffuse positive in the spindled cells,and EMA was positive in the peripheral capsule cells and Acinlan staining was positive.Diagnosis:schwannoma.The lesion was surgically excised.There was no recurrence after 3 months of follow-up.
