摘　　要：目的探讨自适应调强放疗(ART)对局部晚期宫颈癌的治疗效果及不良反应发生率。方法选取80例宫颈癌局部晚期患者,按抽签法随机分为对照组(常规放疗,40例)与干预组(ART,40例)。治疗后对比2组的疾病控制率、客观缓解率、肿瘤标志物水平、不良反应率、危及器官剂量学水平与生活质量评分。结果与对照组比较,干预组的疾病控制率(95.00%)及客观缓解率(85.00%)均更高(P<0.05)。与对照组比较,干预组治疗后的血清癌胚抗原水平及血清鳞癌抗原水平均明显更低(P<0.05)。干预组的各项不良反应发生率较对照组更低(P<0.05),干预组治疗的危及器官(膀胱、小肠、直肠)剂量学水平较对照组均更低(P<0.05)。干预组经过治疗后所评估的各项生活质量评分较对照组更高(P<0.05)。结论ART应用于宫颈癌局部晚期患者治疗的效果显著,对疾病进展的控制效果佳,能有效地降低血清中的肿瘤标志物水平,缓解病情,减少不良反应,改善其生活质量,值得推广应用。Objective To explore the therapeutic effect and incidence of adverse reactions of adaptive intensity modulated radiotherapy(ART)on locally advanced cervical cancer.Methods 80 locally advanced cervical cancer patients were selected and randomly divided into the control group(conventional radiotherapy,40 cases)and the intervention group(ART,40 cases)by drawing lots.After treatment,the disease control rate,objective response rate,tumor marker level,adverse reaction rate,organ threatening dosimetry level,and quality of life score of the 2 groups were compared.Results Compared with the control group,the intervention group had a higher disease control rate(95.00%)and a higher objective response rate(85.00%)(P<0.05).Compared with the control group,the intervention group had significantly lower serum carcinoembryonic antigen levels and serum squamous cell carcinoma antigen levels after treatment(P<0.05).The incidence of various adverse reactions in the intervention group was lower than that in the control group(P<0.05),and the dosage levels of organs at risk(bladder,small intestine,rectum)treated in the intervention group were lower than those in the control group(P<0.05).The quality of life scores evaluated by the intervention group after treatment were higher than those of the control group(P<0.05).Conclusion The application of ART in the treatment of locally advanced cervical cancer patients has a significant effect,with good control of disease progression.It can effectively reduce the levels of tumor markers in the serum,alleviate the condition,reduce adverse reactions,and improve their quality of life.It is worth promoting and applying.
