参芪苡术汤对中晚期胃癌患者肿瘤标志物和生活质量的影响研究

摘　　要：目的观察参芪苡术汤对中晚期胃癌患者肿瘤标志物和生活质量的影响。方法将94例中晚期胃癌患者按照随机数字表法分为2组,对照组47例予以多西他赛+氟尿嘧啶+顺铂(DCF)化疗治疗,治疗组47例在对照组治疗基础上予参芪苡术汤治疗。2组均以21天为1个疗程,治疗3个疗程。比较2组治疗前后症状评分、肿瘤标志物[癌胚抗原(CEA)、糖类抗原199(CA199)、糖类抗原724(CA724)]、免疫功能指标(补体C_(3)、补体C_(4)、CD40细胞)、美国东部肿瘤协作组(ECOG)评分、治疗安全性,并统计2组疗效。结果治疗组总有效率82.98%(39/47),对照组总有效率63.83%(30/47),治疗组疗效优于对照组(P<0.05)。2组治疗后各项症状评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后CEA、CA199、CA724水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后补体C_(3)、补体C_(4)、CD40细胞水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后ECOG评分均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。治疗组不良反应发生率19.15%(9/47),对照组不良反应发生率14.89%(7/47),2组不良反应发生率比较差异无统计学意义(P>0.05)。结论参芪苡术汤治疗中晚期胃癌患者有良好的效果,可改善患者的症状,降低肿瘤标志物水平,提升免疫功能,降低ECOG评分,治疗安全性良好。Objective To observe the effect of Shenqi Yizhu Decoction on tumor markers and quality of life of patients with advanced gastric cancer(AGC).Methods A total of 94 AGC patients were randomly assigned to treatment group(n=47)or control group(n=47).All patients were managed by chemotherapy with docetaxel+cisplatin+5-fluorouracil(DCF)regimen,and those in treatment group were additionally treated with Shenqi Yizhu Decoction.The treatment continued for 3 courses(1 course for 21 days),the aim was to compare traditional Chinese medicine(TCM)syndrome score,tumor markers(carcinoembryonic antigen[CEA],carbohydrate antigen 199[CA199],and carbohydrate antigen 724[CA724]),immune function indexes(complement C_(3),complement C_(4),CD40 cell),Eastern Cooperative Oncology Group(ECOG)score.The curative effect and safety were assessed.Results The overall effective rate in the treatment group was better than that of the control group(82.98%[39/47]vs 63.83%[30/47],P<0.05).After treatment,TCM symptom scores in the both groups were significantly decreased(P<0.05),the decrease was notable in the treatment group compared with the control group(P<0.05).CEA,CA199 and CA724 in the both groups were significantly decreased(P<0.05),the decreases in the treatment group were more common than the control group(P<0.05).The complement levels of C_(3),C_(4),and CD40 in the both groups were significantly decreased(P<0.05),which were significantly lower in the treatment group relative to the control group(P<0.05).ECOG scores in the both groups were significantly decreased(P<0.05),the decrease in the treatment group was more prevalent compared to the control group(P<0.05).The incidence of adverse reactions in the treatment group was comparable to that in the control group(19.15%[9/47]vs 14.89%[7/47],P>0.05).Conclusion For AGC patients,Shenqi Yizhu Decoction has good therapeutic effects,which can improve TCM syndromes,reduce tumor marker levels,improve immune function,decrease ECOG scores,and with good treatment safety.
