摘　　要：目的:本文介绍一种改良式微创上颌窦底内提升术,并评估其临床疗效。方法:收集上颌窦底内提升术种植病例45例,上颌后牙剩余骨量高度2.9~6 mm,患者依手术方式不同分为对照组和观察组,对照组采用骨凿冲顶法,观察组为环切式微创术,即:大直径骨钻、低转速、大扭矩钻孔法,推挤上颌窦底壁向上移位及增强种植体初期稳定性。两组患者术后6个月行种植体上部结构修复。结果:观察组术后满意度优于对照组;影像学测量及临床检查发现,两组患者临床疗效差异无统计学意义。结论:采用不翻瓣、不缝合、不植骨的改良式微创上颌窦底提升术,具有手术创伤小、患者体验佳、减少操作步骤、节省手术费用等优点。Objective:To introduce a minimally invasive method for sinus floor elevation through a circum‐ferential incision,aiming to simplify the operation steps and reduce postoperative trauma.Methods:A total of forty‐five cases of sinus floor elevation for dental implant,with residual bone height(RBH)at a range of 2.9‐6 mm,were collected and randomly divided into the control and observation groups.Topic osteotome technique was used in the control group,and minimally invasive surgery with circum‐ferential incision was applied in the observation group.The special measures included the use of a lowspeed,high‐torque,and large‐diameter bone drill that pushes the floor of the maxillary sinus upward,increasing the primary stability of the implant.After 6 months,superstructures were restored in both groups.Results:The observation group showed a higher postoperative satisfaction rate as compared with control group by postoperative surveys,but no difference in efficacy was found between the two groups after radiological and clinical examinations.Conclusion:This one‐stage approach without incision,flap,suture,or bone grafting is less invasive,provides a better patient experience,reduces operative steps,and saves surgical costs.
