溃疡性上消化道出血应用内镜治疗的疗效及凝血功能各指标的变化研究

摘　　要：目的探讨溃疡性上消化道出血采用消化内镜治疗的临床效果及凝血功能各指标的变化。方法选取2020年6月—2023年1月新乡市中心医院诊治的62例溃疡性上消化道出血患者,按治疗方法不同分组,药物治疗的31例作为对照组,消化内镜治疗的31例作为观察组,对相关的治疗指标、凝血功能指标在治疗前后的变化、治疗后症状改善时长、临床效果以及不良反应发生情况等进行比较。结果(1)观察组输血量、止血时间和住院时间为(384.88±38.65)mL、(10.38±2.47)h和(9.32±1.03),均短于对照组(P<0.05);(2)两组治疗前凝血功能指标比较无统计学意义(P>0.05),观察组治疗后凝血酶原时间(PT)、凝血酶时间(TT)和活化部分凝血活酶时间(APTT)为(9.35±0.46)s、(12.86±0.91)s和(22.56±1.32)s,均短于对照组(P<0.05),观察组纤维蛋白原(Fbg)水平(3.95±0.43)g/L,低于对照组(P<0.05);(3)观察组临床症状改善时长统计指标为(4.39±1.12)h、(27.79±5.93)h、(1.66±0.43)d及(2.85±0.67)d,均短于对照组(P<0.05);(4)观察组总有效率92.5%,高于对照组的75.00%(P<0.05)。结论消化内镜治疗溃疡性上消化道出血的效果明显,可以有效改善患者的凝血系统功能,缩短患者止血时间、住院时间和症状改善时间。Objective To investigate the clinical effect of digestive endoscopy in the treatment of hemorrhage gastrointestinal upper ulcerative.Methods Sixty-two patients with ulcerative upper digestive tract hemorrhage treated in our hospital from June 2020 to January 2023 were selected.According to different treatment methods,31 patients treated with drugs were taken as the control group,and 31 patients treated with digestive endoscopy were taken as the observation group.The changes of relevant therapeutic indexes,coagulation function indexes before and after treatment,symptom improvement time after treatment,clinical effects and adverse reactions were compared.Resultss①Theblood transfusion volume,hemostatic time and hospital stay of the observation group were(384.88±38.65)mL,(10.38±2.47)h and(9.32±1.03),which were shorter than those of the control group(P<0.05).The coagulation function indexes of the two groups were compared before treatment(P>0.05).After treatment,the PT,TT and APTT of the observation group were(9.35±0.46)s,(12.86±0.91)s and(22.56±1.32)s,which were all shorter than those of the control group(P<0.05),and the Fbg level of the observation group was(3.95±0.43)g/L.Lower than control group(P<0.05);③The statistical indexes of improvement time of clinical symptoms in observation group were(4.39±1.12)h,(27.79±5.93)h,(1.66±0.43)d and(2.85±0.67)d,which were shorter than those of control group(P<0.05).The total effective rate of observation group was 92.5%higher than that of control group(75.00%)(P<0.05).Conclusion Digestive endoscopy is the effective in treatment of gastrointestinal upper ulcerative bleeding,can improve effectively function of the coagulation system,shorten the time of hemostasis,hospital stay and symptom improvement.
