摘　　要：目的探索新型免疫检查点人内源性逆转录病毒-H长末端重复关联蛋白2(HHLA2)在人食管胃交界腺癌组织中的表达及其临床意义。方法选取2016年3月至2018年6月安阳市肿瘤医院收治的食管胃交界腺癌患者126例作为研究对象,采用免疫组化染色检测食管胃交界腺癌和癌旁正常组织中HHLA2表达,随访终点为患者死亡。采用COX回归风险模型进行多因素生存分析,采用Kaplan-Meier法绘制生存曲线。结果HHLA2在食管胃交界腺癌组织中阳性表达率为68.9%(78/126),高于癌旁正常组织的21.1%(27/126)(χ^(2)=42.465,P<0.001)。多因素COX分析显示,TNM分期、HHLA2表达是患者总生存期的独立预后因素(HR=4.217,P<0.001;HR=2.467,P=0.003)。高表达HHLA2和低表达HHLA2的食管胃交界腺癌患者5 a生存率分别为24.4%和56.3%(χ^(2)=21.140,P<0.001)。结论HHLA2在食管胃交界腺癌组织中表达升高,且与患者总生存期存在相关性;高表达HHLA2和TNM分期进展是导致患者总生存期缩短的独立危险因素。Objective To explore the expression of human endogenous retrovirus-H long terminal repeat-associating 2(HHLA2)in human esophageal gastric junction adenocarcinoma and its clinical significance.Methods The 126 patients with esophageal gastric junction adenocarcinoma admitted to our hospital from March 2016 to Jue 2018 were randomly selected as the study subjects.Immunohistochemical staining was used to detect the expression of HHLA2 in the tumor tissue and corresponding adjacent tissues.The follow-up endpoint was patient death.A COX regression risk model was used for multivariate survival analysis,and the Kaplan Meier method was used to plot the survival curve.Results The positive expression rate of HHLA2 in patients with esophageal gastric junction adenocarcinoma was 68.9%(78/126),and the positive expression rate of HHLA2 in adjacent tissues was 21.1%(27/126),with a significant statistical difference(χ^(2)=42.465,P<0.001).Multivariate COX regression analysis showed that high expression of TNM stage and HHLA2 was associated with overall survival in patients with esophageal gastric junction adenocarcinoma(HR=4.217,P<0.001;HR=2.467,P=0.003).The overall survival rate of patients with high expression of HHLA2 decreased compared to those with low expression(24.4%and 56.3%;χ^(2)=21.140,P<0.001).Conclusion HHLA2 is highly expressed in gastric esophageal junction adenocarcinoma,which is significantly correlated with overall survival of patients.In addition,high expression of HHLA2 and TNM stage is an independent risk factor for shortening overall survival of patients.
