“叩齿调气”动留针术治疗恢复期中重度贝尔面瘫的临床观察

摘　　要：目的观察动留针术治疗恢复期中重度贝尔面瘫的疗效。方法60例患者随机分为对照组与动留针组各30例,对照组参照指南所推荐的恢复期贝尔面瘫针刺方法治疗,动留针组在此基础上加用动留针术。两组均接受每日1次的针刺治疗,每6次为1个疗程,连续治疗4个疗程,中病即止。比较两组治疗结束后、治疗结束后1个月的House-Brackmann面神经分级量表(HB)评分、平均痊愈时间、临床痊愈率、口角歪斜症状的改善程度以及面部残疾指数(FDI)以及不良事件发生情况。结果治疗后及随访时,动留针组各时间段痊愈率、痊愈疗程均优于对照组(P<0.05)。两组双侧口角到门齿中缝的距离差值均缩短,动留针组更为明显(P<0.05)。两组FID评分均明显改善,动留针组改善更为明显(P<0.05)。结论针刺治疗可安全、有效改善恢复期、单侧、中重度贝尔面瘫患者面部表情肌的运动功能。“动留针术”有进一步缩短病程、提高痊愈率、改善躯体功能及社会生活功能的疗效,今后可进一步扩大样本量进行临床研究予以证实。Objective:To observe the efficacy of dynamic retention of needles in the treatment of moderate-tosevere Bell's palsy during the recovery period.Methods:A total of 60 patients were randomly divided into the control group and the acupuncture group.The control group was treated with acupuncture for convalescent Bell's palsy recommended by the guidelines,and the acupuncture group was treated with"dynamic retention of needles"on this basis.Both groups received acupuncture treatment once a day,with 1 course of treatment every 6 times and totally 4 courses of continuous treatment.The House-Brackmann(HB)Facial Nerve Grading Scale scores,the mean healing time,the clinical healing rate,the degree of improvement in the symptoms of orofacial askewness,and the Facial Disability Index(FDI),as well as the incidence of adverse events were compared between the two groups at the end of the treatment,and 1 month after the end of the treatment.Results:After treatment and during follow-up,the cure rate and treatment duration until cure in the dynamic needle retention group were better than those in the control group at different time periods(P<0.05).The differences in the distances between the bilateral oral corners and the midline of the incisors in both two groups were shortened,and it was more significant in the dynamic needle rentention group(P<0.05).Both groups showed significant improvement in FID scores,with the improvement being more significant in the dynamic needle retention group(P<0.05).Conclusion:Acupuncture can safely and effectively improve the motor function of facial expression muscles in recovering,unilateral,moderate-to-severe Bell's palsy patients."Dynamic retention of needles"can further shorten the course of the disease,increase the healing rate,and improve somatic and social life functions.Its efficacy can be further confirmed by clinical studies with expanded sample size in the future.
