针刺治疗急性期贝尔氏面瘫的临床观察

摘　　要：目的通过肌骨超声技术观察针刺治疗急性期贝尔氏面瘫的临床疗效。方法采用随机数字表法将60例急性期贝尔氏面瘫患者分为观察组与对照组各30例。观察组接受常规针刺治疗,对照组接受常规药物治疗。每周进行4次针刺,治疗持续至病程第28天结束。观察并比较治疗前后两组的总有效率、肌骨超声下面神经直径变化、House-Brackmann(H-B)量表分级和面部残疾指数(FDI)等指标。结果治疗后,观察组总有效率为93.33%,明显高于对照组的76.67%(P<0.05)。此外,治疗后两组的面神经直径均减小,观察组的患侧面神经减小幅度更显著(P<0.05)。面部神经功能评分(H-B)和面部残疾指数(FDI)在两组中均有改善(P<0.05),而观察组的改善幅度均优于对照组(P<0.05)。结论针刺治疗急性期贝尔氏面瘫疗效确切,能明显改善临床症状。此外,本研究验证了面神经直径能作为贝尔氏面瘫病证程度的客观评价。Objective:To investigate the clinical efficacy of acupuncture in the treatment of Bell's palsy in acute stage using musculoskeletal ultrasonography.Methods:60 patients with Bell's palsy in acute stage were divided into an observation group and a control group according to random number table method,with 30 cases in each.The observation group received conventional acupuncture treatment,while the control group received conventional drug treatment.Acupuncture was administered four times a week,continuing until the end of the 28th day of illness.The total effective rate,changes in nerve diameter under musculoskeletal ultrasound,House-Brackmann(H-B)scale grading,and Facial Disability Index(FDI)were observed and compared between the two groups before and after treatment.Results:After treatment,the total effective rate for the observation group was 93.33%,which was significantly higher than 76.67%of the control group(P<0.05).Additionally,there was a reduction in facial nerve diameter in both groups after treatment;however,this reduction was more significant in the affected facial nerve of the observation group(P<0.05).Both groups showed improvement in H-B score and FDI score after treatment(P<0.05),with greater improvement observed in the observation group compared with the control group(P<0.05).Conclusion:Acupuncture demonstrates precise efficacy in treating Bell's palsy in acute stage and can significantly improve clinical symptoms.Furthermore,this study confirms that measuring diameter of facial nerves can serve as an objective evaluation method for assessing degree of Bell's palsy.
