摘 要:目的分析血浆外泌体长链非编码RNA(LncRNA)SFTA1P和miR-483-3p在乙型肝炎病毒(HBV)相关性肝癌(HCC)的表达水平,评估其诊断价值。方法选择2022年1月—2024年1月本院收治的58例HBV相关性HCC患者作为HCC组;将同期收治的55例HBV患者作为HBV组;另选同期50名体检健康者作为对照组。运用纳米颗粒跟踪分析系统和蛋白质印迹分析鉴定血浆外泌体,并采用RT-qPCR检测外泌体LncRNA SFTA1P和miR-483-3p水平,分析二者水平分别与HCC临床病理特征之间的关系,Pearson法分析LncRNA SFTA1P和miR-483-3p表达水平的相关性,受试者工作特征(ROC)曲线分析LncRNA SFTA1P和miR-483-3p对HCC患者诊断的敏感性和特异性。结果外泌体LncRNA SFTA1P与miR-483-3p表达水平呈负相关(r=-0.7277,P<0.001)。与对照组相比,HBV、HCC组LncRNA SFTA1P水平依次增高,而miR-483-3p水平依次降低,差异均有统计学意义(P<0.05)。LncRNA SFTA1P、miR-483-3p表达水平与TNM分期,肿瘤分化程度、淋巴结转移、AFP及AST有关(P<0.05),而与年龄、肿瘤大小、ALT、γ-GGT无关(P>0.05)。LncRNA SFTA1P和miR-483-3p单独诊断HCC时的曲线下面积(AUC)分别为0.886、0.851,敏感度分别为86.20%、89.70%,特异度分别为78.00%、70.00%;联合诊断的AUC为0.957,敏感度和特异度分别为94.80%、82.00%。结论血浆外泌体LncRNA SFTA1P和miR-483-3p的联合诊断可提高HBV相关性HCC的诊断率,为其诊断及后续治疗提供新的研究思路。Objective To analyze the expression levels of plasma exosomal long non-coding RNA(LncRNA)SFTA1P and miR-483-3p in hepatitis B virus(HBV)-related hepatocellular carcinoma(HCC),and to evaluate their diagnostic value.Methods 58 patients with HBV-related HCC who were admitted to the Second Affiliated Hospital of Wannan Medical College from January 2022 to January 2024 were enrolled in the study as the HCC group,55 patients with HBV were enrolled as the HBV group,and 50 healthy individuals were enrolled as the control group.Identification of plasma exosomes using nanoparticle tracking analysis system and Western blot analysis,and the levels of LncRNA SFTA1P and miR-483-3p in exosomes were detected with RT-qPCR,the relationship between the levels of the two and the clinical pathological characteristics of the patients suffering from HCC was analyzed.Pearson method was used to analyze the correlation between the expression levels of LncRNA SFTA1P and miR-483-3p,and receiver operating characteristic(ROC)curve was used to analyze the sensitivity and specificity of LncRNA SFTA1P and miR-483-3p in the diagnosis of HCC patients.Results The levels of exosomal LncRNA SFTA1P and miR-483-3p were negatively correlated(r=-0.7277,P<0.001).Compared with the control group,the levels of exosomal LncRNA SFTA1P in the HBV and HCC group increased sequentially,while the levels of miR-483-3p decreased sequentially,with statistically significant differences(P<0.05).The expression level of LncRNA SFTA1P and miR-483-3p were correlated with the TNM stage,degree of tumor differentiation,lymph node metastasis,AFP,and AST(P<0.05),but were not related to age,tumor size,ALT andγ-GGT(P>0.05).The area under the curve(AUC)of exosomal LncRNA SFTA1P and miR-483-3p for diagnosing HCC separately was 0.886 and 0.851,respectively.The sensitivity was 86.20%and 89.70%,respectively.The specificity was 78.00%and 70.00%,respectively.The AUC of the combined diagnosis was 0.957,and the sensitivity and the specificity were 94.80%and 82.00%,respectively.Conclusions
