摘　　要：目的分析高压氧治疗对脑膜瘤患者术后复发的影响。方法对2011年1月至2019年1月青岛大学附属医院神经外科收治的196例术后病理学确诊为脑膜瘤的患者进行随访,采用单因素Kaplan-Meier生存分析探讨脑膜瘤术后复发因素及高压氧治疗对脑膜瘤术后复发的影响。结果196例患者随访过程中失访8例、死亡18例,随访中位时间[85(68,103)]个月;有效随访的170例患者中,出现脑膜瘤复发27例。单因素Kaplan-Meier生存分析显示,肿瘤最大直径和病理亚型是影响肿瘤复发的因素(P<0.05),而是否接受高压氧治疗对肿瘤复发没有影响;高压氧治疗组较常规治疗组总体生存期延长(P=0.032),2组肿瘤最大直径和病理亚型等一般资料差异无统计学意义;高压氧治疗组内,复发组患者年龄明显大于未复发组(P=0.028)。结论脑膜瘤术后给予患者高压氧治疗对脑膜瘤术后复发无影响,本研究对临床应用起到一定指导作用。Objective To analyze the effect of hyperbaric oxygen on postoperative recurrence of meningioma.Methods A total of 196 patients who were diagnosed pathologically as meningioma and underwent surgical operation at the Department of Neurosurgery of the Affiliated Hospital of Qingdao University from January 2011 to January 2019 were followed up.The Kaplan-Meier survival analysis was used to explore the factors of postoperative recurrence of meningioma and the effect of hyperbaric oxygen therapy on postoperative recurrence of meningioma.Results During the follow-up of the 196 patients,eight cases were lost to follow up and 18 cases died.The median follow-up time was[85(68,103)]months.Among the 170 valid follow-up cases,27 cases had meningioma recurrence.The Kaplan-Meier survival analysis showed that maximum tumor diameter and pathological subtype were factors influencing tumor recurrence(P<0.05).Whether undergoing hyperbaric oxygen therapy had no effect on tumor recurrence.The overall survival time of the hyperbaric oxygen therapy group was longer than that of the conventional treatment group(P=0.032).There was no significant difference in the maximum tumor diameter and pathological subtypes between the two groups.In the hyperbaric oxygen therapy group,the age of patient in the recurrence group was significantly higher than that in the non-recurrence group(P=0.028).Conclusion Hyperbaric oxygen therapy has no effect on postoperative recurrence of meningioma,which plays a guiding role in its clinical application.
