温经活血通络方外用治疗白蛋白紫杉醇所致阳虚血瘀型周围神经病变的临床研究

摘　　要：目的 探讨温经活血通络方外用治疗白蛋白紫杉醇所致阳虚血瘀型周围神经病变(OIPN)的效果。方法 选取2022年6月至2023年8月醴陵市中医院由白蛋白紫杉醇化疗导致OIPN且中医辨证符合阳虚血瘀型的患者60例为研究对象,采用随机数表法将其分为对照组(n=30,予以甲钴胺胶囊治疗)和治疗组(n=30,予以甲钴胺胶囊+温经活血通络方治疗),两组均治疗14 d。评估两组疗效、毒性副反应、中医症候评分和不良反应情况。结果 治疗组总有效率高于对照组,差异有统计学意义(P <0.05)。治疗后,治疗组毒性副反应及中医证候评分均低于对照组,差异有统计学意义(P <0.05)。两组患者均未出现明显不良反应。结论 对紫杉类药物所致阳虚血瘀型OIPN患者予以温经活血通络方外用治疗,其临床疗效较佳,可改善患者临床症状,且无明显不良反应。Objective To explore the effect of external application of Wenjing Huoxue Tongluo Formula in the treatment of oxaliplatin-induced peripheral neuropathy(OIPN)caused by albumin paclitaxel with yang deficiency and blood stasis type.Methods A total of 60 patients with OIPN caused by albumin paclitaxel chemotherapy at Liling Hospital of Traditional Chinese Medicine from June 2022 to August 2023 were selected as the study subjects,and their traditional Chinese medicine diagnosis was consistent with the yang deficiency and blood stasis type.The patients were randomly divided into a control group(n=30,treated with methylcobalamin capsules)and a treatment group(n=30,treated with methylcobalamin capsules and Wenjing Huoxue Tongluo Fang)by a random number table method.Both groups were treated for 14 days.The efficacy,toxic side effects,traditional Chinese medicine syndrome scores,and adverse reactions were evaluated in both groups.Results The total effective rate of the treatment group was higher than that of the control group,with a statistically significant difference(P<0.05).After treatment,the toxic side effects and traditional Chinese medicine syndrome scores of the treatment group were lower than those of the control group,and the difference was statistically significant(P<0.05).There were no significant adverse reactions observed in the two group.Conclusion The Wenjing Huoxue Tongluo Fang is effective in treating OIPN patients with yang deficiency and blood stasis caused by paclitaxel drugs.It can improve clinical symptoms and has no significant adverse reactions.
