摘 要:目的探讨MRI定量增强参数对乳腺癌的诊断价值及其与人表皮生长因子受体2(HER-2)、血管生成素2(Ang-2)表达的关系。方法选取80例乳腺癌患者为研究对象,比较不同临床分期以及淋巴结转移情况的MRI定量增强参数[速率常数(K_(ep))、血管外细胞外间隙容积分数(V_(e))、容量转移常数(K^(trans))],并采用受试者工作特征(ROC)曲线分析MRI定量增强参数对HER-2、Ang-2表达的诊断价值。结果Ⅱ期组Kep、K^(trans)低于Ⅲ期组和Ⅳ期组(F=6.322,F=6.676;P<0.05);淋巴结转移组Kep、K trans高于淋巴结未转移组(t=4.265,t=4.557;P<0.05);HER-2阳性组Kep、K^(trans)高于HER-2阴性组(t=3.988,t=4.616;P<0.05);ROC曲线分析显示,Kep、K trans联合对乳腺癌患者HER-2表达诊断的曲线下面积(AUC)0.887,特异度0.811,敏感度0.889;Ang-2阳性组Kep、K^(trans)高于Ang-2阴性组(t=2.534,t=3.189;P<0.05);ROC曲线分析显示,K_(ep)、K^(trans)联合对乳腺癌患者Ang-2表达诊断的AUC0.767,特异度0.867,敏感度0.569。结论MRI定量增强参数可提高对乳腺癌以及HER-2、Ang-2表达诊断价值,有利于指导临床治疗方案的选择。Objective To explore the diagnostic value of MRI quantitative enhancement parameters in breast cancer and their rela-tionships with human epidermal growth factor receptor-2(HER-2)and angiopoietin-2(Ang-2).Methods A total of 80 patients with breast cancer were selected as the study objects.The MRI quantitative enhancement parameters[rate constant(K_(ep)),extravascular extracellular space volume fraction(V_(e)),volume transfer constant(K^(trans))]of all patients with different clinical staging and lymph node metastasis were compared,respectively.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of MRI quantitative enhancement parameters on the expression of HER-2 and Ang-2.Results Kep and Kirns in stageⅡgroup were significantly lower than those in stageⅢ and Ⅳgroups(F=6.322,F=6.676;P<0.05).K_(ep) and K^(trans) in lymph node metas-tasis group were significantly higher than those in lymph node non-metastasis group(1=4.265,t=4.557;P<0.05).K_(ep) and K^(trans) in HER-2 positive group were significantly higher than those in HER-2 negative group(t=3.988,t=4.616;P<0.05).ROC curve analysis showed that the area under the curve(AUC)of the combination of K_(ep) and K^(trans) for the diagnosis of HER-2 expression in breast cancer patients was 0.887,with specificity of 0.811 and sensitivity of 0.889,respectively.Kep and Kiruns in Ang 2 positive group were signifi-cantly higher than those in Ang-2 negative group(t=2.534,t=3.189;P<0.05).ROC curve analysis showed that the AUC of the com-bination of K.,and Kitrs for the diagnosis of Ang-2 expression in breast cancer patients was 0.767,with specificity of 0.867 and sensitivity of 0.569,respectively.Conclusion MRI quantitative enhancement parameters can improve the diagnostic value of breast cancer and the expression of HER-2 and Ang-2,which are beneficial to guide the selections of clinical therapeutic regimens.
