舒肝解郁胶囊联合常规用药治疗老年抑郁症患者的临床价值

摘　　要：目的探讨舒肝解郁胶囊联合常规用药治疗老年抑郁症患者的临床价值。方法选取2021年7月至2022年7月抚州市第三医院收治的70例老年抑郁症患者作为研究对象,采用电脑抽签法随机分为对照组与观察组,每组35例。对照组予以常规用药(阿戈美拉汀+文拉法辛)治疗,观察组在对照组基础上联合疏肝解郁胶囊治疗。比较两组临床疗效、心理状态、血清炎症因子水平、神经递质指标水平及不良反应发生情况。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后1、3周,观察组汉密顿抑郁量表(HAMD)评分低于对照组,差异有统计学意义(P<0.05)。治疗前,两组肿瘤坏死因子-α(TNF-α)、白细胞介素-2(IL-2)、C反应蛋白(CRP)及多巴胺(DA)、5-羟色胺(5-HT)、去甲肾上腺素(NE)水平比较差异无统计学意义;治疗后,两组TNF-α、IL-2、CRP水平均低于治疗前,DA、5-HT、NE水平均高于治疗前,且观察组TNF-α、IL-2、CRP水平低于对照组,DA、5-HT、NE水平高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论舒肝解郁胶囊联合常规用药(阿戈美拉汀+文拉法辛)治疗老年抑郁症临床效果显著,可抑制患者炎症反应,改善神经介质水平,有效控制抑郁症状,安全性较高,值得临床推广应用。Objective To explore the clinical value of Shugan Jieyu capsule combined with conventional medicine in the treatment of elderly de-pression.Methods 70 elderly patients with depression admitted to the Third Hospital of Fuzhou from July 2021 to July 2022 were selected as the study subjects,and they were randomly divided into the control group and the observation group by computer lottery method,with 35 cases in each group.The control group was treated with conventional medication(agomelatine+venlafaxine),and the observation group was treated with Shugan Jieyu capsule on the basis of the control group.The clinical efficacy,psychological state,serum inflammatory factors,neurotransmitter indexes and the occurrence of adverse reactions were compared between the two groups.Results The total treatmen effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).1 and 3 weeks after treatment,the score of Hamilton depression scale(HAMD)in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in the levels of tumor necrosis factor-α(TNF-α),interleukin-2(IL-2),C-reactive protein(CRP)and dopamine(DA),5-hydroxytryptamine(5-HT),norepinephrine(NE)between the two groups;after treatment,the levels of TNF-α,IL-2 and CRP of the two groups were lower than those before treatment,and the levels of DA,5-HT and NE were higher than those before treatment,and the levels of TNF-α,IL-2 and CRP in the observation group were lower than those in the control group,and the levels of DA,5-HT and NE were higher than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the inci-dence of adverse reactions between the two groups.Conclusion Shugan Jieyu capsule combined with conventional medicine(argomepratine+ven-lafaxine)has significant clinical effect in the treatment of elderly d
