中药调控核因子κB信号通路防治慢性萎缩性胃炎的研究进展

摘　　要：慢性萎缩性胃炎(CAG)是一种消化系统常见疾病,因其症状隐匿且易反复发作,并伴有一定癌变风险,近年来在临床越来越受到重视。在CAG进展过程中,核因子κB(NF-κB)信号通路的异常转导是炎症产生并级联扩大的中心环节。相关研究表明,活化的NF-κB在CAG中高度表达,并通过调控细胞促炎因子、血管生成因子和抗凋亡蛋白等靶基因的活性,特异性地调控炎症反应,影响胃黏膜细胞正常的增殖与凋亡周期,促进CAG的发生发展。诸多药理研究表明,中药防治CAG的效果显著,不仅能减少炎性细胞浸润,抑制胃黏膜细胞凋亡,还能延缓或逆转黏膜病变,基于NF-κB信号通路探寻中药干预CAG进程的研究也逐渐增加。在此,笔者通过检索2007—2023年中国知网(CNKI)、万方医学网、维普、PubMed等中英文数据库中的文献,就中药调控NF-κB信号通路防治CAG的国内外研究现状进行综述,以期为进一步治疗CAG提供药物应用与研发的参考。Chronic atrophic gastritis(CAG)is a common disease of the digestive system.It has received increasing attention in recent years because of the insidious and recurrent symptoms and the risk of evolving into gastric cancer.During the progression of CAG,the abnormal transduction of nuclear factor(NF)-κB signaling pathway is central to the generation and cascade expansion of inflammation.Studies have shown that activated NF-κB is highly expressed in CAG and specifically regulates the inflammatory response by modulating the activities of target genes encoding pro-inflammatory cytokines,angiogenic factors,and anti-apoptotic proteins,thus affecting the normal proliferation and apoptotic cycles of gastric mucosal cells and promoting the development of CAG.Pharmacological studies have shown that Chinese medicine is effective in preventing and treating CAG,not only reducing inflammatory cell infiltration and inhibiting apoptosis of gastric mucosal cells,but also delaying or reversing mucosal lesions.Increasing studies have been conducted to explore the interventional effect of Chinese medicine in CAG development via the NF-κB signaling pathway.Here,we retrieved the related articles from the China Knowledge Infrastructure(CNKI),Wanfang Med Online,VIP,and Pub Med with the time interval of 2007–2023 to review the research progress in the modulation of the NF-κB signaling pathway by Chinese medicine in the prevention and control of CAG,aiming to provide a reference for the research and development of drugs for treating CAG.
