摘 要:慢性萎缩性胃炎(CAG)被认定为胃癌的癌前状态,该病在全球的发病率不断攀升,对其发病机制研究、生物学诊断技术及治疗药物的革新势在必行。相关消化道菌群微生态的研究极有可能推动这一革新。随着对幽门螺杆菌(Hp)研究的深入,人们发现单独干预Hp并不能完全延缓萎缩性胃炎进展,由此口腔菌群、胃内非Hp菌群及肠道菌群组成的“口-胃-肠”菌群微生态轴在CAG中的表观特征逐步得到重视。本文通过查阅中国知网、万方、维普、PubMed等数据库自2000—2023年的相关文献,筛出48篇目标文献,并将对口腔及舌苔菌群、胃内非Hp菌群和肠道菌群3个方面与CAG的关系进行回顾,同时探讨中医药基于调节菌群对CAG治疗的可行性,展望“胆汁酸-菌群轴”在未来CAG的菌群研究中将发挥的重要作用。Chronic atrophic gastritis(CAG)has been recognized as a precancerous state of gastric cancer,and the increasing incidence of this disease worldwide has necessitated innovations in its pathogenesis research,biological diagnostic techniques,and therapeutic agents.Research on the microecology of relevant digestive tract bacteria is likely to contribute to this innovation.With the deepening of research on helicobacter pylori(Hp),it has been found that intervention in Hp alone cannot completely slow down the progression of atrophic gastritis.Therefore,the epigenetic characteristics of the"oral-gastro-intestinal"microecological axis composed of oral bacteria,gastric non-Hp bacteria,and intestinal bacteria in CAG have been gradually emphasized.This paper reviewed the relevant literature from CNKI,Wanfang,VIP,Pub Med,and other databases from 2000 to 2023 and screened 48 target papers.This paper reviewed the relationship of CAG with oral and tongue bacteria,gastric non-Hp bacteria,and intestinal bacteria.It discussed the feasibility of applying traditional Chinese medicine(TCM)for the treatment of CAG based on the regulation of bacteria and expected that the"bile acid-bacteria axis"will play an important role in the future study of the bacteria of CAG.
