半量CAG方案联合地西他滨治疗老年初诊急性髓系白血病患者的效果

摘　　要：目的:观察半量CAG方案联合地西他滨治疗老年初诊急性髓系白血病患者的效果。方法:选取2020年1月至2023年1月于该院就诊的108例老年初诊急性髓系白血病患者进行前瞻性研究,按随机数字表法将其分为对照组和观察组各54例。对照组予以地西他滨治疗,观察组在对照组基础上联合半量CAG方案治疗,比较两组临床缓解率、住院时间、治疗前后T细胞亚群指标水平和治疗期间不良反应发生率。结果:观察组临床缓解率为72.22%(39/54),高于对照组的40.74%(22/54),差异有统计学意义(P<0.05);观察组住院时间短于对照组,差异有统计学意义(P<0.05);治疗后,两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:半量CAG方案联合地西他滨治疗老年初诊急性髓系白血病患者可提高临床缓解率,缩短住院时间,降低T细胞亚群指标水平,效果优于单纯地西他滨治疗。Objective:To observe effects of half quantity CAG regimen combined with Decitabine in treatment of elderly patients with newly diagnosed acute myeloid leukemia.Methods:A prospective study was conducted on 108 elderly patients with acute myeloid leukemia who were admitted to the hospital from January 2020 to January 2023.According to the random number table method,they were divided into control group and observation group,54 cases in each group.The control group was treated with Decitabine,while the observation group was treated with half quantity CAG regimen on the basis of that of the control group.The clinical remission rate,the hospitalization time,the T cell subset levels before and after the treatment,and the incidence of adverse reactions during hospitalization were compared between the two groups.Results:The clinical remission rate of the observation group was 72.22%(39/54),which was higher than 40.74%(22/54)of the control group,and the difference was statistically significant(P<0.05).The hospitalization time of the observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of CD3^(+),^(+)CD4 and CD4^(+)/CD8^(+)in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Half quantity CAG regimen combined with Decitabine in the treatment of the elderly patients with acute myeloid leukemia can improve the clinical remission rate,shorten the hospitalization time,and reduce the levels of T cell subsets.Moreover,it is superior to single Decitabine treatment.
