隐形矫治器与Damon自锁托槽治疗成人上颌牙弓轻中度牙性狭窄的疗效比较

摘　　要：目的:采用CBCT对比评价隐形矫治器及Damon自锁托槽治疗成人上颌牙弓轻中度牙性狭窄后牙及牙槽骨的变化及效果。方法:纳入隐形矫治器组33例与Damon矫治器组32例患者,于治疗前后分别拍摄CBCT,采用Invivo5软件测量治疗前后上颌第一前磨牙间及第一磨牙间的宽度、牙倾斜度、颊侧牙槽骨厚度。结果:与治疗前相比,两组患者治疗后第一前磨牙间和第一磨牙间宽度、牙倾斜度、颊侧牙槽骨厚度变化均有统计学差异(P<005);治疗后Damon矫治器组与隐形矫治器组第一前磨牙间宽度、第一前磨牙倾斜度、第一磨牙倾斜度有统计学差异(P<0.05),上颌第一磨牙间宽度及颊侧牙槽骨厚度无统计学差异(P>0.05)。结论:两种矫治器对于上颌牙弓轻中度牙性狭窄的成人患者均有一定疗效。隐形矫治器组在上颌第一前磨牙间扩弓效果更好,扩弓后上颌第一前磨牙、第一磨牙牙倾斜程度更小。Objective To analyze the changes in teeth and alveolar bone of the maxillary dental arch in adults with mild to moderate dental arch constriction after treatment with invisible aligners and Damon self-ligating brackets using cone-beam computed tomography(CBCT)and the efficacy.Methods The study included 33 patients in the invisalign group and 32 in the Damon group.CBCT images were taken before and after treatment,and the width between the first premolars and first molars,tooth inclination,and buccal alveolar bone thickness were measured using Invivo5 both before and after treatment.Results Compared with the conditions before treatment,the changes in the width between the first premolars and first mo⁃lars,tooth inclination,and buccal alveolar bone thickness after treatment showed significant difference in both groups(P<0.05).Differences were observed in the width of the first premolars and the inclination of the first premolars and first molars between Damon group and invisalign group after treatment(P<0.05).However,no difference was found in the width be⁃tween the first molars and buccal alveolar bone thickness(P>0.05).Conclusion Both orthodontic appliances are effective for adults with mild to moderate dental arch constriction of the maxilla.The invisalign group had better expansion effects be⁃tween the first premolars,and smaller inclination in the first premolar and first molar after expansion.
