摘　　要：目的 探讨乳腺浸润性导管癌(breast invasive ductal carcinoma, BIDC)患者MMP2和p120ctn表达与临床病理指标及预后的关系。方法 应用免疫组织化学EliVision^(TM) plus两步法检测80例BIDC患者手术标本及30例癌旁乳腺组织MMP2和p120ctn的表达,分析MMP2和p120ctn表达与患者年龄、TNM分期、分化等级、淋巴结宏转移、术后5年内复发转移5项指标的关系。结果 BIDC及癌旁乳腺组织MMP2阳性率分别为78.8%(63/80)和16.7%(5/30),p120ctn阳性率分别为45.0%(36/80)和86.7%(26/30),差异均有统计学意义(P<0.05)。MMP2和p120ctn表达均与年龄无关(P>0.05)。在TNM(T_(3)+T_(4))期、低分化、有淋巴结宏转移和5年内复发转移患者中MMP2高表达,p120ctn低表达,两者呈负相关(r=-0.46)。结论 BIDC发生发展与MMP2和p120ctn显著相关,MMP2高表达或(和)p120ctn低表达均预示BIDC发展快、分化低、转移早、预后差。Objective To investigate the MMP2 and p120ctn expression in breast invasive ductal carcinoma(BIDC)and the relationship with clinicopathologic parameters and prognosis.Methods Immunohistochemical EliVision TM plus two-step method was used to detect the expression of MMP2 and p120ctn in 80 BIDC tissues who underwent resection and 30 paracancerous tissues.The relationships between the expression of MMP2 and p120ctn and the clinicopathologic parameters including patient age,TNM staging,differentiated degree,lymph node macrometastasis,recurrence and metastasis in 5 years after surgery were studied.Results In BIDC tissues and paracancerous tissues,the positive rates of MMP2 were 78.8%and 16.7%,p120ctn were 45.0%and 86.7%,respectively,with statistically significant differences(P<0.05).There was no significant difference in the expression of MMP2 and p120ctn between the age of two groups(P>0.05).In patients with high TNM staging(T_(3) and T_(4)),poor differentiation,lymph node macrometastasis,and recurrence and metastasis within 5 years of follow-up,MMP2 was highly expressed,but p120ctn was lowly expressed.A significantly inverse relationship was observed between the expression of MMP2 and p120ctn in BIDC tissues(r=-0.46).Conclusion MMP2 and p120ctn were significantly associated with the development of BIDC,and either high expression of the MMP2 or low expression of the p120ctn predicted rapid progression,poor differentiation,early metastasis and poor prognosis.
