摘 要:目的 探讨宫颈癌前病变的发生与高危型人乳头瘤病毒(HR-HPV)负荷量的关系。方法 回顾性分析2022-03—2023-03月于北京市延庆区医院病理科接受HR-HPV分型定量检测及阴道镜活检的233例宫颈癌前病变患者的临床资料,其中低级别鳞状上皮内病变(low-grade squamous intraepithelial lesion, LSIL)(LSIL或CINⅠ级)130例,高级别鳞状上皮内病变(high-grade squamous intraepithelial lesion, HSIL)95例,HSIL中包括CINⅡ级59例和CINⅢ级36例,宫颈原位腺癌8例。结果 233例宫颈活检标本HR-HPV总检出率为87.0%,检出HR-HPV亚型15种,检出率排前4位分别为HPV16(25.3%)、HPV52(17.6%)、HPV58(15.0%)以及HPV51(10.3%);HSIL组HR-HPV负荷量明显高于LSIL组(P<0.05);CINⅠ级、CINⅡ级及CINⅢ级的病毒负荷量呈显著递增趋势(P<0.05)。结论 HR-HPV感染与宫颈癌前病变的发生密切相关,HR-HPV负荷量检测可作为宫颈癌前病变、宫颈癌筛查及病情追踪监测的一项客观指标。Objective To investigate the relationship between the occurrence of cervical precancerous lesions and the viral load of high-risk human papillomavirus(HR-HPV).Methods Clinical data of 233 patients with cervical precancerous lesions who underwent HR-HPV typing quantitative testing and vaginal biopsy at the Pathology Department of Yanqing District Hospital in Beijing from March 2022 to March 2023 were retrospectively analyzed.Among them,there were 130 cases of low-grade squamous intraepithelial lesions(LSIL or CIN grade Ⅰ),95 cases of high-grade squamous intraepithelial lesions(HSIL),59 cases of CIN grade Ⅱ and 36 cases of CIN grade Ⅲ in HSIL,and 8 cases of cervical adenocarcinoma in situ.Results The total detection rate of HR-HPV in 233 cases of cervical biopsy specimens was 87.0%,with 15 subtypes of HR-HPV detected.The top four detection rates were HPV16(25.3%),HPV52(17.6%),HPV58(15.0%),and HPV51(10.3%),respectively;HR-HPV load in HSIL group was significantly higher than that in LSIL group(P<0.05);viral load of CIN Ⅰ,CIN Ⅱ,and CIN Ⅲ showed a significant increasing trend(P<0.05).Conclusion HR-HPV infection is closely related to the occurrence of cervical precancerous lesions,and HR-HPV load testing can serve as an objective indicator for cervical precancerous lesions,cervical cancer screening,and disease tracking monitoring.
