摘 要:目的 探讨扩散加权成像(DWI)定量参数ADCmin、ADC mean与浸润性乳腺癌预后因子的关系。方法 回顾性分析158例浸润性乳腺癌患者的临床资料,所有患者入院后均接受了乳腺MR检查,并行乳腺穿刺活检术或乳腺癌根治术病理检查确诊。统计患者肿瘤生物学预后因子(Ki-67、ER、PR)表达情况,比较不同表达状态的ADC_(min)、ADC_(mean)值的差异。绘制受试者工作特征(ROC)曲线评价ADC_(min)、ADC_(mean)值对不同因子表达状态的预测价值。结果 Ki-67高表达的ADC_(min)值及ADC_(mean)值均低于低表达,差异有统计学意义(P<0.05),ER、PR阳性表达的ADC_(min)、ADC_(min)值高于阴性表达,ADCmin值差异有统计学意义(P<0.05)。ROC曲线显示,ADC_(min)值预测Ki-67表达状态的最大曲线下面积(AUC)为0.906,ADC mean值预测Ki-67表达状态的最大AUC为0.889;ADC_(min)与ADC_(mean)预测ER表达状态的最大AUC分别为0.682、0.57和预测PR表达状态的最大分别ACU为0.639、0.585;ADC_(min)值预测Ki-67、ER、PR表达状态AUC的均高于ADC_(mean)值,差异有统计学意义(P<0.05)。结论 DWI参数ADC_(min)值、ADC_(mean)值有助于评估浸润性乳腺癌Ki-67、ER、PR的表达状态,ADC_(min)值对Ki-67、ER、PR表达状态的预测价值较大。Objective To investigate the relationship between diffusion-weighted imaging(DWI) quantitative parameters ADCmin and ADCmean and biological prognostic factors in invasive breast cancer.Methods The clinical data of 158 patients with invasive breast cancer were retrospectively analyzed.All patients received MR examination after admission and were confirmed by biopsy or radical mastectomy.The expression of biological prognostic factors(Ki-67,ER,PR) was analyzed and the ADC_(min)、ADC_(mean) values of different expression states were compared.Receiver operating characteristic(ROC) curve was drawn to evaluate the predictive value of the ADC_(min),ADC_(mean) values to different factor expression states.Results the ADCmin value and ADC_(mean) value of high expression of Ki-67 were lower than those of low expression,with statistical significance(P<0.05).the ADC_(min)、 ADC_(mean) value of positive expression of ER and PR was higher than that of negative expression,with ADC_(min) statistical significance(P<0.05).The ROC curve showed that the maximum area under the curve(AUC) for the ADC_(min) value to predict Ki-67 expression status was 0.906,and the maximum AUC for the ADCmean value to predict Ki-67 expression status was 0.889.The maximum AUC of ADC_(min) and ADC_(mean) to predict ER expression status were 0.682 and 0.578,respectively.The maximal ACU for predicting PR expression status were 0.639 and 0.585.ADC_(min) value predicted Ki-67,ER and PR expression status of AUC higher than ADCmean value,the difference was statistically significant(P<0.05).Conclusion Preoperative DWI parameters ADCmin and ADCmean values are helpful to evaluate the expression status of Ki-67,ER and PR in invasive breast cancer,and ADC_(min) values are significant in predicting the expression status of Ki-67,ER and PR.
