摘　　要：目的:探讨环状RNA(circular RNA,CircRNA)_0025039及CircRNA_0084043在恶性黑素瘤组织中的表达及其预测淋巴结转移的价值。方法:选取59例恶性黑素瘤患者和35例色素痣患者,采用实时荧光定量PCR法检测恶性黑素瘤及色素痣CircRNA_0025039和CircRNA_0084043的表达水平,分析CircRNA_0025039和CircRNA_0084043表达水平与恶性黑素瘤患者临床特征的关系。应用受试者工作特征(ROC)曲线分析CircRNA_0025039和CircRNA_0084043预测恶性黑素瘤淋巴结转移的价值。Pearson相关进行相关性分析。结果:恶性黑素瘤组织中CircRNA_0025039(5.06±2.42 vs.1.47±0.58)和CircRNA_0084043(2.60±1.15 vs.0.87±0.31)表达水平均高于色素痣组织(P<0.001)。恶性黑素瘤患者CircRNA_0025039和CircRNA_0084043表达水平升高与组织分级Ⅲ~Ⅳ级、肿瘤部位、低分化及淋巴结转移有关(P<0.001)。ROC曲线显示,CircRNA_0025039和CircRNA_0084043二项联合预测恶性黑素瘤淋巴结转移的曲线下面积为0.961(95%CI:0.902~0.997),其敏感度为97.4%,特异度为85.2%。相关分析显示,恶性黑素瘤组织CircRNA_0025039水平与CircRNA_0084043呈正相关(r=0.825,P<0.001)。结论:恶性黑素瘤组织中CircRNA_0025039和CircRNA_0084043水平明显升高,二项联合对预测恶性黑素瘤淋巴结转移具有一定的价值。Objective:To investigate the expression of CircRNA_0025039 and CircRNA_0084043 in malignant melanoma tissues and their value in predicting lymph node metastasis.Methods:A total of 59 patients with malignant melanoma and 35 patients with pigmented nevus were selected as research objects,and the expression levels of CircRNA_0025039 and CircRNA_0084043 in malignant melanoma and nevus tissues were detected by real-time quantitative PCR.The relationship between the expression level of CircRNA_0025039 and CircRNA_0084043 and the clinical characteristics of patients with malignant melanoma was analyzed.The value of CircRNA_0025039 and CircRNA_0084043 in predicting lymph node metastasis of malignant melanoma was analyzed by receiver operating characteristic(ROC)curve.Pearson correlation was used for correlation analysis.Results:The expression levels of CircRNA_0025039(5.06±2.42 vs.1.47±0.58)and CircRNA_0084043(2.60±1.15 vs.0.87±0.31)in malignant melanoma tissues were significantly higher than those in nevus tissues(P<0.001).The increased expression levels of CircRNA_0025039 and CircRNA_0084043 in malignant melanoma patients were associated with tissue gradeⅢ-Ⅳ,tumor location,low differentiation and lymph node metastasis(P<0.001).ROC curve showed that the area under the curve of combined CircRNA_0025039 and CircRNA_0084043 in predicting lymph node metastasis of malignant melanoma was 0.961(95%CI:0.902-0.997),with a sensitivity 97.4%and a specificity of 85.2%.Correlation analysis showed that CircRNA_0025039 levels were positively correlated with CircRNA_0084043 in malignant melanoma tissues(r=0.825,P<0.001).Conclusion:The levels of CircRNA_0025039 and CircRNA_0084043 in malignant melanoma tissues were significantly increased,and their combination had a high accuracy in predicting lymph node metastasis of malignant melanoma.
