摘　　要：目的 了解能量多普勒超声(PDU)联合双参数非增强磁共振成像(bp-MRI)诊断前列腺癌(PCa)的价值。方法 收集进行前列腺结节筛查的患者的资料。先进行PDU检查,再行bp-MRI,随后穿刺活检。所有患者共129例,PDU分级为0~3级;bp-MRI评分为1~5分。运用受试者工作特征(ROC)曲线分别绘制PDU、bp-MRI、PDU+bp-MRI曲线图,比较曲线下面积(AUC),了解其诊断PCa的效能。结果 穿刺病理良性48例、恶性81例。PDU筛查PCa的AUC为0.804,bp-MRI的AUC为0.812,PDU+bp-MRI的AUC为0.915(与PDU比较,z=3.035,P=0.002 4;与bp-MRI比较,z=2.632,P=0.015 9)。以超声血流信号2级、MRI评分4分为截断值,PDU+bp-MRI筛查PCa的灵敏度为95%,特异度为79.2%,准确度为89.1%。结论 PDU联合bp-MRI可以提高PCa的诊断效能。Objective To explore the value of power Doppler ultrasound(PDU)combined with bi-parametric non-contrast magnetic resonance imaging(bp-MRI)in diagnosis of prostate cancer(PCa).Methods The data of pa-tients with prostate nodules screened by were collected.PDU examination was performed first,followed by bp-MRI,finally with puncture biopsy.A total of 129 patients were diagnosed.Blood flow signal was grade as O-3 on PDU;the bp-MRI score was 1-5.Receiver operating characteristic(ROC)curve was plotted according to the results of PDU,bp-MRI,and PDU+bp-MRI,and the area under the curve(AUC)were compared to evaluate their diagnostic efficacy for PCa.Results A total of 48 cases were benign and 81 cases were malignant in puncture pathology.The AUC of PDU and bp-MRI were 0.804,and 0.812 respectively.The AUC for PDU combined with bp-MRI is 0.915(compared with PDU,z=3.035,P=0.0024;compared with bp-MRI,z=2.632,P=0.0159).Using level 2 of ultrasonic blood flow signal and score 4 of MRI as cutoff values,the sensitivity,specificity,and accuracy of PDU+bp-MRI in diagnosing PCa were 95%,79.2% and 89.1%,respectively.Conclusions Power Doppler ultrasound com-bined with bp-MRI can improve the diagnostic efficacy in PCa.
