丁苯酞联合脉血康胶囊对动脉粥样硬化性缺血性脑卒中患者神经功能及血液流变学的影响

摘　　要：目的 探究丁苯酞联合脉血康胶囊对动脉粥样硬化性缺血性脑卒中(IS)患者神经功能及血液流变学的影响。方法 前瞻性随机选取安阳市中医院2020年12月至2023年7月收治的86例动脉粥样硬化性IS患者纳入研究。将其采用随机数字表法分成两组,各43例。对照组接受丁苯酞治疗,观察组接受丁苯酞联合脉血康胶囊治疗。比较两组神经功能、血液流变学、血清指标及用药安全性。结果 治疗后,两组美国国立卫生研究院卒中量表(NIHSS)评分、中枢神经特异蛋白(S100β)、超敏C反应蛋白(hs-CRP)、低氧诱导因子-1α(HIF-1α)水平均下降,观察组低于对照组(P<0.05)。治疗后,两组纤维蛋白原(Fib)、总胆固醇(TC)、血浆黏度、甘油三酯(TG)、全血高切黏度、低密度脂蛋白(LDL-C)、全血低切黏度水平均下降,观察组低于对照组,两组高密度脂蛋白(HDL-C)水平升高,观察组高于对照组(P<0.05)。两组不良反应差异无统计学意义(P>0.05)。结论 丁苯酞联合脉血康胶囊用于动脉粥样硬化性IS临床治疗,可通过改善神经功能、血液流变学及血脂水平,促进患者病情稳定,同时用药安全可控。Objective To investigate the effects of butylphthalide combined with maexuekang capsule on neurological function and hemorheology in patients with atherosclerotic ischemic stroke(IS).Methods A total of 86 patients with atherosclerotic IS admitted to Anyang Hospital of Traditional Chinese Medicine from December 2020 to July 2023 were prospectively selected and randomly included in the study.They were divided into 2 groups by random number table method,43 cases in each group.The control group was treated with butylphthalide,and the observation group was treated with butylphthalide combined with maexuekang capsule.The neurological function,hemorheology,serum indexes,blood lipid levels and drug safety were compared between the two groups.Results After treatment,the National Institutes of Health stroke scale(NIHSS)score,central nervous system specific proteinβ(S100β),high-sensitivity C-reactive protein(hs-CRP)and hypoxia-inducible factor-1α(HIF-1α)levels of the two groups all decreased,and the observation group was lower than the control group(P<0.05).After treatment,two groups of fibrinogen(Fib),total cholesterol(TC),triglyceride(TG),plasma viscosity,whole blood high shear viscosity,low-density lipoprotein(LDL-C),whole blood low shear viscosity level decreased,the observation group was lower than the control group,two groups had elevated levels of high-density lipoprotein(HDL-C),the observation group was higher than control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Butylphthalide combined with maexuekang capsule in the clinical treatment of atherosclerotic IS can improve neurological function,hemorheology and blood lipid levels,promote the stability of the patient’s condition,and the medication is safe and controllable.
