摘　　要：目的:观察冰冻切片联合快速免疫组织化学检测在乳腺癌前哨淋巴结转移诊断中的应用价值。方法:收集2022年3月-2023年10月贵州医科大学附属白云医院收治的190例乳腺癌患者病理组织标本,均采取冰冻切片、快速免疫组织化学法、石蜡切片进行前哨淋巴结活检,以石蜡切片病理结果作为“金标准”,评估冰冻切片、快速免疫组织化学法以及两者联合检测对乳腺癌前哨淋巴结转移的诊断效能。结果:190例患者石蜡切片病理结果显示,乳腺癌前哨淋巴结转移56例。冰冻切片联合快速免疫组织化学检测诊断乳腺癌前哨淋巴结转移的准确率、灵敏度、阴性预测值高于冰冻切片、快速免疫组织化学法,诊断乳腺癌前哨淋巴结转移的特异度、阳性预测值低于冰冻切片、快速免疫组织化学法,差异有统计学意义(P<0.05)。结论:冰冻切片联合快速免疫组织化学检测在乳腺癌前哨淋巴结转移诊断中的临床价值较高,能够提高诊断准确率、灵敏度、阴性预测。Objective:To observe the value of frozen section combined with rapid immunohistochemical detection in the diagnosis of sentinel lymph node metastasis of breast cancer.Methods:Pathological tissue specimens of one hundred and ninety breast cancer patients admitted to Guizhou Medical University Affiliated Baiyun Hospital from March 2022 to October 2023 were collected,and sentinel lymph node biopsy was performed by frozen section,rapid immunohistochemical method and paraffin section.Using the pathological results of paraffin section as the"gold standard",the diagnostic efficacy of frozen section,rapid immunohistochemistry and combined detection of sentinel lymph node metastasis in breast cancer was evaluated.Results:The pathological results of paraffin sections of 190 patients showed that there were 56 cases of breast cancer with sentinel lymph node metastasis;the accuracy,sensitivity and negative predictive value of frozen section combined with rapid immunohistochemistry in the diagnosis of sentinel lymph node metastasis of breast cancer were higher than those of frozen section combined with rapid immunohistochemistry;the specificity and positive predictive value in the diagnosis of sentinel lymph node metastasis of breast cancer were lower than those of frozen section and rapid immunohistochemistry,and the differences were statistically significant(P<0.05).Conclusion:Frozen section combined with rapid immunohistochemical detection has high clinical value in the diagnosis of sentinel lymph node metastasis of breast cancer,which can improve the diagnostic accuracy,sensitivity and negative prediction.
