青少年非自杀性自伤与自杀未遂的关联:焦虑、抑郁的调节作用

摘　　要：目的:探讨非自杀性自伤与自杀未遂的关联,并考察焦虑和抑郁在其中的调节作用。方法:采用非自杀性自伤、焦虑、抑郁自评量表及相关临床访谈工具对就诊于综合医院心理门诊的青少年患者进行评估与诊断。共收集到非自杀性自伤青少年患者277名,其中有自杀未遂经历者110例,无自杀未遂经历者167例。结果:非自杀性自伤青少年患者的自伤方式数量(OR=0.627,P=0.037)、击打自己(OR=4.017,P=0.010)和在皮肤或指甲下插入物体(OR=4.892,P=0.008)的自伤行为与自杀未遂存在显著关联;非自杀性自伤的频率和焦虑的交互作用对自杀未遂的预测作用显著(B=0.081,P=0.011);“击打自己”自伤方式与焦虑(B=0.085,P=0.030)和抑郁(B=0.090,P=0.009)之间的交互作用显著。结论:特定的自伤方式如击打自己、皮肤下插入物体对自杀未遂有直接影响;焦虑、抑郁对非自杀性自伤频率、击打自己自伤方式与自杀未遂的关系有一定的调节作用。Objective:To explore the relationship between non-suicidal self-injury(NSSI)and suicide attempts,and the moderating effect of anxiety and depression.Methods:The non-suicidal self-injury,anxiety,and depressive self-rat⁃ing scales and related clinical interview tools were used to evaluate and diagnose adolescent patients from the outpatient de⁃partment of clinical psychology in general hospitals.A total of 277 adolescent patients who had non-suicidal self-injury for more than 1 day in the past year were collected,including 110 with suicide attempt and 167 without suicide attempt.Re⁃sults:The number of NSSI methods(OR=0.627,P=0.037),the behaviors of self-hitting(OR=4.017,P=0.010)and inserting objects under skin or nails(OR=4.892,P=0.008)were significantly associated with suicide attempts.The interaction be⁃tween the frequency of NSSI and medium-high level of anxiety had a significant predictive effect on suicide attempts(B=0.081,P=0.011).There was a significant interaction between the ways of hitting oneself and the medium-high level of anxi⁃ety(B=0.085,P=0.030)and the medium-high level of depression(B=0.090,P=0.009).Conclusion:Specific forms of selfinjury,such as hitting oneself and inserting objects under the skin,have direct impact on suicide attempts,while anxiety and depression have certain moderating effects on the relationship between the frequency of NSSI,or hitting oneself and sui⁃cide attempts.
