摘 要:目的分析本体感觉训练联合踝足支具训练对偏瘫患者下肢功能的影响。方法以2021年3月至2022年12月厦门市康复医院接诊200例偏瘫患者为观察对象,采用随机数字表法进行分组,对照组102例患者接受本体感觉训练,观察组98例患者接受本体感觉训练联合踝足支具训练,分析对患者下肢运动功能(FMA)评分与平衡功能(BBS)评分及表面肌电图的影响。结果训练后,观察组FMA评分与BBS评分均高于对照组(P<0.05);观察组股四头肌、腓肠肌、胫骨前肌等肌电积分值(iEMG)均高于对照组,方根振幅(RMS)均低于对照组(P<0.05);观察组股四头肌、腘绳肌肌力均高于对照组(P<0.05)。结论本体感觉训练联合静态踝足支具训练有助于提高偏瘫患者FMA评分、BBS评分及股四头肌、腓肠肌、胫骨前肌iEMG,降低RMS,提高股四头肌、腘绳肌肌力,改善患者下肢功能。Objective To analyze the effect of proprioceptive training combined with ankle foot support training on lower limb function in hemiplegic patients.Methods A total of 200 hemiplegic patients admitted to Xiamen Rehabilitation Hospital from March 2021 to December 2022 were selected as the observation subjects.Random number table method was used for grouping.A control group of 102 patients received proprioceptive training,while an observation group of 98 patients received proprioceptive training combined with ankle and foot support training.The effects of lower limb motor function(FMA)score,balance function(BBS)score,and surface electromyography were analyzed.Results After training,the FMA and BBS scores of the observation group were higher than those of the control group(P<0.05);The integrated electromyographic(iEMG)values of the quadriceps femoris,gastrocnemius muscle,tibialis anterior muscle and other muscles in the observation group were higher than those in the control group,while the root square amplitude(RMS)was lower than that in the control group(P<0.05);The lower limb muscle strength of the quadriceps and hamstring muscles in the observation group was higher than that in the control group(P<0.05).Conclusions The combination of proprioceptive training and static ankle foot brace training can help improve the FMA score,BBS score,and iEMG of the quadriceps,gastrocnemius,and tibialis anterior muscle in hemiplegic patients,reduce RMS,enhance the strength of the quadriceps,hamstring muscle,and improve lower limb function in patients.
