角巩膜缘与角膜切口超声乳化术治疗老年白内障疗效对比

摘　　要：目的 对比角巩膜缘与角膜切口超声乳化术治疗老年白内障的临床疗效。方法 随机将80例老年白内障患者分为2组。2组均行超声乳化术,对照组采用角膜切口,观察组采用角巩膜缘切口。比较2组上皮修复时间、切口修复时间、裸眼视力、最佳矫正视力(BCVA)、角膜散光度、手术源性角膜散光度、泪囊破裂时间(BUT)、角膜荧光素染色评分(FLCs)、泪液分泌试验(SIt)水平以及术后并发症发生率。结果 观察组上皮修复、切口修复时间均早于对照组(P<0.05);观察组术后1周、2周、4周时裸眼视力、BCVA水平以及BUT、Sit水平均高于对照组(P<0.05);观察组术后1周、2周、4周时角膜散光度、手术源性角膜散光度以及FLCs水平水平均低于对照组(P<0.05);2组术后并发症发生率(18%与12%)差异无统计学意义(P>0.05)。结论 角巩膜缘切口超声乳化术治疗老年白内障临床疗效较好,患者术后角膜散光度较低,且视力、泪膜功能稳定性恢复较好。Objective To compare the effect of phacoemulsification with corneal incision and corneal incision in the treatment of senile cataracts.Methods A total of 80 patients with senile cataracts were randomly divided into 2 groups.Both groups underwent phacoemulsification,the corneal incision was used in the control group,and the marginal corneoscleral incision was used in the observation group.Epithelial repair time,incision repair time,naked eye visual acuity,best corrected visual acuity(BCVA),corneal astigmatism,operative keratogenic astigmatism,break-up time(BUT),corneal fluorescein staining score(FLCs),Schirmer test(SIt)and postoperative complications rate were compared between the two groups.Results The time of epithelial repair and incision repair in the observation group was earlier than that in the control group(P<0.05),while eye visual acuity,BCVA,BUT,Sit levels in the observation group were higher(P<0.05).Corneal astigmatism,operative corneal astigmatism,and FLC levels in the observation group were lower than those in the control group at 1 week,2 weeks,and 4 weeks after surgery(P<0.05).The incidence of postoperative complications in two groups(18%vs 12%)was no difference(P>0.05).Conclusion The clinical effect of phacoemulsification with marginal corneoscleral incision in the treatment of senile cataracts is good,the postoperative corneal astigmatism of patients is lower,and the stability of visual acuity and tear film function is better.
