摘　　要：目的通过对胃癌及癌旁正常组织氨肽酶P2(XPNPEP2)、埃兹蛋白(EZRIN)的表达情况,探讨二者在胃癌发生、发展时的表达情况及临床意义。方法选取2020年2月至2022年12月于枣庄市立医院胃肠外科就诊的患者,就诊前无任何治疗措施,经组织病理诊断胃癌组96例及正常对照组64例。应用免疫组织化学方法检测胃癌及癌旁正常组织中的XPNPEP2和EZRIN的表达水平。结合临床资料分析其表达情况与患者年龄、性别、肿瘤大小、TNM分期、浸润转移情况的关系。结果病例组XPNPEP2、EZRIN阳性表达率分别为67.71%、63.54%,均高于对照组的26.56%、29.69%,差异有统计学意义(P<0.05);在不同TNM分期、是否发生浸润转移的患者中XPNPEP2阳性表达率组间比较,差异有统计学意义(P<0.05);XPNPEP2、EZRIN阳性表达率与患者年龄、性别、肿瘤大小无关,差异无统计学意义(P>0.05)。结论胃癌组织中XPNPEP2、EZRIN蛋表达水平升高,二者高表达与胃癌TNM分期、肿瘤浸润转移显著相关,在胃癌发生、侵袭、转移过程中发挥了重要作用,可能提示预后不良。Objective To explore the clinical significance during the occurrence and development of gastric cancer by investigating the amino peptidase P2(XPNPEP2)and Ezrin(EZRIN)expressions in gastric tumor tissues and adjacent normal tissues.Methods Patients who visited the Department of Gastroenterology at Zaozhuang Municipal Hospital from February 2020 to December 2022 and have not received any treatment before were selected.After histopathological diagnosis,96 cases of gastric tumor tissues and 64 cases of normal control group were identified.Immunohistochemistry was conducted to detect the expression levels of XPNPEP2 and EZRIN in gastric tumor issues and adjacent normal tissues.The correlations between XPNPEP2 and EZRIN expressions and patient age,gender,tumor size,TNM staging,tumor infiltration and metastasis were analyzed based on clinical data.Results The positive expression rates of case group of XPNPEP2 and EZRIN were 67.71%and 63.54%,respectively,both higher than in the control group(26.56%and 29.69%,respectively),with statistically significant differences(P<0.05).There were statistically significant differences in the expression rate of XPNPEP2 between groups of patients at different TNM stages and patients that occurred or had not occurred infiltration and metastasis(P<0.05).The positive expression rates of XPNPEP2 and EZRIN were not significantly correlated with patient age,gender,and tumor size,without statistically significant differences(P>0.05).Conclusion The high expression levels of XPNPEP2 and EZRIN in gastric tumor tissues are significantly correlated with TNM staging and tumor infiltration and metastasis in gastric cancer patients,which may indicate poor prognosis and can play an important role in the diagnosis of occurrence,invasion and metastasis of gastric cancer.
