缺血性脑卒中患者脑电图特点及与认知障碍的相关性研究

摘　　要：目的:探究缺血性脑卒中(Ischemic stroke,IS)患者脑电图(Electroencephalogram,EEG)特点及与认知障碍的相关性。方法:回顾性分析2021年5月~2023年5月福州大学附属省立医院收治的70例IS患者的临床资料,患者均行蒙特利尔认知量表(Montreal Cognitive Assessment,MoCA)评估和EEG检查,据评估结果分为认知障碍组(45例)和认知正常组(25例),探究EEG检查结果与认知障碍的相关性。结果:认知障碍组的视空间与执行功能、命名、记忆、注意力、语言、抽象思维、延迟回忆、定向力各维度评分和MoCA总分均低于认知正常组,差异有统计学意义(P<0.05);EEG结果中认知障碍组正常状态低于认知正常组,中度异常、重度异常状态和总体异常程度均高于认知正常组(P<0.05);异常EEG特点为弥漫性θ波活动增加,低至高波幅δ波阵发,α波呈不规则、不稳定状态且节律变慢,出现阵发性尖波和棘波;IS患者MoCA量表评分随EEG异常程度的增加而降低,组间两两比较,差异有统计学意义(P<0.05)。结论:IS认知障碍患者EEG多表现弥漫性θ波活动增加,低至高波幅δ波阵发,α波呈不规则、不稳定状态且节律变慢,出现阵发性尖波和棘波,且其认知障碍程度与EEG异常程度存在显著的正相关性。thinking,delayed recall,orientation and total score of MoCA in the cognitively impaired group were lower than those in the cognitively normal group,with statistical significance(P<0.05).The normal state of the cognitive impairment group was lower than that of the cognitive normal group,and the moderate abnormal state,severe abnormal state and overall abnormal degree were higher than that of the cognitive normal group(P<0.05).The abnormal EEG was characterized by increased diffuseθwave activity,low to high amplitudeδwave array,irregular and unstableαwave,slow rhythm,and episodic spikes and spikes.The score of MoCA scale in IS patients decreased with the increase of EEG abnormality,and the difference between groups was statistically significant(P<0.05).Conclusion:The EEG of patients with IS cognitive impairment mostly showed increased diffuseθwave activity,low to high amplitudeδwave array,irregular and unstableαwave with slow rhythm,and episodic sharp wave and spike wave,and the degree of cognitive impairment was significantly positively correlated with the degree of EEG abnormality.
