摘 要:目的旨在探讨食管癌患者代谢指标的变化及其与食管癌发生的相关性。方法选取2020年1月—2023年6月青岛市黄岛区中心医院食管癌患者80例,与同期健康体检者81例,分别纳入癌症组和对照组,收集他们的人口学特征及临床资料,检测血清总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)、高密度脂蛋白(high-density lipoprotein,HDL)、低密度脂蛋白(low-density lipoprotein,LDL)、脂蛋白a(lipoprotein-a,LP-a)、尿酸(uric acid,UA)、血钙(calcium,Ca)等代谢指标水平,对比上述指标在2组间的差异,探讨血脂代谢水平、UA、Ca与食管癌的相关性。结果癌症组与对照组体质量指数(BMI)、TC、TG、LDL、Ca无统计学差异,与癌症组比差异有统计学意义的是饮酒史、吸烟史、LP-a、HDL、UA(P<0.05)。二元Logistic回归分析发现,食管癌的发生与吸烟有很大的关系,同时发现LP-a升高和UA降低与食管癌的发生发展密切相关。结论食管癌患者存在明显血脂代谢异常,其中吸烟史和LP-a是食管癌的危险因素,UA为食管癌的保护因素。Objective To investigate the changes of metabolic indexes in patients with esophageal cancer and their correlation with the occurrence of esophageal cancer.Methods 80 patients with esophageal cancer treated in Central Hospital of Huangdao District from January 2020 to June 2023,and 81 healthy individuals who underwent physical examinations during the same period were selected and included in the cancer group and the control group,respectively.Their demographic characteristics and clinical data were collected.The levels of serum total cholesterol(TC),triglyceride(TG),high density lipoprotein(HDL),low-density lipoprotein(LDL),lipoprotein-A(LP-a),uric acid(UA),blood calcium(Ca)and other metabolic indicators were measured.The differences of the above indexes between the two groups were compared,and the correlation between the level of lipid metabolism,UA and Ca and esophageal cancer were explored.Results There was no statistically significant difference in body mass index(BMI),TC,TG,LDL and Ca between the two groups,but there were statistically significant differences in drinking history,smoking history,LP-a,HDL,and UA between the two groups(P<0.05).Binary logistic regression analysis found that the occurrence of esophageal cancer was closely related to smoking,and it was also found that increased LP-A and decreased UA were closely related to the occurrence and development of esophageal cancer.Conclusion Patients with esophageal cancer have obvious abnormal lipid metabolism.Smoking history and LP-a are risk factors for esophageal cancer,and UA is a protective factor for esophageal cancer.
