摘　　要：目的分析宫颈高危型人乳头瘤病毒(HPV)感染患者接受普罗雌烯阴道胶丸联合抗HPV凝胶敷料治疗对患者阴道菌群、相对发光单位(RLU)/临床阈值(CO)水平、局部免疫炎症的影响。方法选择2022年10月至2023年10月杭州市余杭区第三人民医院收治的244例宫颈HPV感染患者为研究对象,按患者接受治疗的不同方法分为观察组(122例)和对照组(122例)。对照组接受干扰素治疗,观察组在此基础上应用普罗雌烯阴道胶丸联合抗HPV凝胶敷料治疗,比较2组患者治疗后HPV转阴率、临床有效率、阴道菌群、RLU/CO水平、局部免疫炎症及病毒载量变化。结果治疗后,观察组与对照组患者转阴率与临床有效率均提升,差异有统计学意义(P<0.05);诊治30 d、60 d和治疗结束停药90 d后观察组转阴率比对照组高,差异有统计学意义(P<0.05)。治疗后,观察组与对照组患者病毒载量均下降,差异有统计学意义(P<0.05);诊治30 d、60 d以及治疗结束停药90 d后观察组病毒载量较对照组低,差异有统计学意义(P<0.05)。观察组治疗后免疫功能优于对照组,差异具有统计学意义(P<0.05)。观察组阴道菌群改善率(82.8%)高于对照组(66.4%),差异具有统计学意义(P<0.05)。结论普罗雌烯阴道胶丸联合抗HPV凝胶敷料能改善HPV患者症状,降低病毒载量,改善阴道菌群、调节局部免疫炎症反应,在防治宫颈HPV感染中具有较好的临床应用价值。Objective To investigate the clinical effects of proestrol vaginal capsules combined with an-tihuman papillomavirus(HPV)gel dressing on vaginal flora,RLU/CO level,and local immune inflammation in pa-tients with cervical HPV infection.Methods A total of 244 cervical HPV infected patients treated from October 2022 to October 2023 were selected as the study subjects.They were divided into an observation group and a control group according to different treatment methods,with 122 cases in each group.The control group was treated with interferon,while the observation group was treated with proestrol vaginal capsules combined with HPV gel dressing(relinetab)on the basis of the control group.The HPV overcast rate,clinical efficiency,vaginal flora,RLU/CO level,local immune inflammation,and changes of viral load in the two groups were observed and compared after the treatment.Results After the treatment,the overcast rate and clinical response rate of the ob-servation group and the control group increased significantly,which were statistically different(P<0.05).After 30 d,60 d,and 90 d of the treatment,the observation group had higher overcast rate than the control group(P<0.05).The viral load between the observation and the control group decreased,which were statistically significant(P<0.05).After 30 d,60 d,and 90 d of the treatment,lower viral load was observed,which was significantly different(P<0.05).The immune function of the observation group was significantly better than that of the control group after the treatment,and the difference was statistically significant(P<0.05).The improvement rate of vaginal microbiota in the observation group(82.8%)was significantly higher than that in the control group(66.4%),and the differ-ence was statistically significant(P<0.05).Conclusion Proestrol vaginal capsules combined with HPV gel dress- ing can significantly improve the symptoms and vaginal flora of HPV patients, reduce the viral load, and regulate local immune inflammatory reaction, which have good clinical application valu
