摘 要:经皮脊柱内镜技术已成为近年骨科脊柱外科分支学科的研究热点,内镜下手术不仅可保证小切口,在脊柱疾病中也具有一定疗效。自同轴脊柱内镜操作系统等经典术式被提出后,各类成熟的微创术式逐渐展开。微创手术在腰椎间盘突出症、腰椎管狭窄症等腰椎退行性疾病(LDD)中应用尤为广泛,经皮脊柱内镜技术的适应证已拓宽至单纯椎弓峡部裂、腰椎滑脱、腰椎退行性侧凸等疾病。目前临床常用的治疗LDD的经皮脊柱内镜技术主要包括全内镜下减压术、内镜辅助下减压术、内镜辅助下融合手术等。经皮脊柱内镜发展前景广阔,但学习曲线陡峭,仍需术中精细操作和积累一定的手术经验。Percutaneous spinal endoscopic technology has become one of the most major research topics in spine surgery in recent years.Endoscopic surgery,while guaranteeing small incision,has a certain effect on spinal diseases.Since the introduction of classical surgeries such as Yeung endoscopic spine system,various mature minimally invasive technologies have gradually unfolded.Minimally invasive surgery is particularly widely used in lumbar degenerative disease(LDD)such as lumbar disc herniation and lumbar spinal stenosis,and the indications for percutaneous spinal endoscopy have been broadened to simple vertebral arch isthmus fissure,lumbar spondylolisthesis,and lumbar degenerative scoliosis.At present,percutaneous spinal endoscopic techniques commonly used in the treatment of LDD mainly include total endoscopic decompression,endoscopic-assisted decompression,endoscopic-assisted fusion surgery,etc.Percutaneous spinal endoscopy has broad prospects for development,but the learning curve is steep,and it still requires delicate intraoperative operations and certain surgical experience.
