摘 要:目的探讨病理特征、CT检查影像特征与肺癌患者表皮生长因子受体(EGFR)突变的关系。方法选取80例肺癌患者,依据EGFR基因突变情况分为突变组和未突变组,比较两组患者的临床特征、病理特征、CT检查影像特征,肺癌患者EGFR基因突变情况的影响因素采用多因素Logistic回归分析。结果EGFR基因突变检测结果显示,80例肺癌患者中,EGFR基因突变49例,未突变31例。突变组与未突变组肺癌患者年龄、胸膜凹陷征、坏死征、毛刺征、分叶征、吸烟指数、病理分型比较,差异均有统计学意义(P﹤0.05)。多因素Logistic回归分析结果显示,胸膜凹陷征、坏死征、毛刺征、分叶征、无吸烟、病理分型为腺泡型均是肺癌患者EGFR基因突变的独立危险因素(P﹤0.05)。结论胸膜凹陷征、坏死征、毛刺征、分叶征、无吸烟、病理分型为腺泡型均是肺癌患者EGFR基因突变的独立危险因素,这对肺癌的诊断和治疗有重要的临床意义。Objective To explore the relationship between pathological features,CT examination imaging features,and epidermal growth factor receptor(EGFR)mutations in lung cancer patients.Method A total of 80 lung cancer pa-tients were selected and divided into mutation group and unmutated group based on EGFR gene mutations.The clinical features,pathological features,and CT examination imaging features of the two groups were compared.The influencing factors of EGFR gene mutations in lung cancer patients were analyzed by multivariate Logistic regression analysis.ReRe-sult The results of EGFR gene mutation detection showed that among 80 lung cancer patients,49 cases had EGFR gene mutations and 31 cases had no mutations.The differences in age,pleural indentation sign,necrosis sign,spicule sign,lob-ulation sign,smoking index,and pathological classification between the mutation group and the unmutated group of lung cancer patients were statistically significant(P<0.05).The multivariate Logistic regression analysis showed that pleural indentation sign,necrosis sign,spicule sign,lobulation sign,non-smoking,and pathological classification as acinar type were all independent risk factors for EGFR gene mutations in lung cancer patients(P<0.05).Conclusion Pleural indenta-tion sign,necrosis sign,spicule sign,lobulation sign,non-smoking,and pathological classification as acinar type are all in-dependent risk factors for EGFR gene mutations in lung cancer patients,which have important clinical significance for the diagnosis and treatment of lung cancer.
