Vitapex与三联抗生素糊剂应用于牙髓血运重建术中的临床疗效

摘　　要：目的探讨Vitapex、三联抗生素糊剂(TAP)应用于牙髓血运重建术中的临床疗效。方法选取2019年4月至2020年10月枣庄市口腔医院收治的100例根尖周炎或年轻恒牙牙髓炎患儿作为研究对象,采用随机数字表法分对照组与观察组,各50例。对照组采用ATP作为根管消毒剂,观察组采用Vitapex作为根管消毒剂,比较两组临床疗效、牙根生长发育状况、治疗满意度及不良反应发生率。结果观察组治疗总有效率为96.00%,高于对照组的78.00%,差异有统计学意义(P<0.05)。观察组牙体变色率为2.00%,低于对照组的18.00%,差异有统计学意义(P<0.05)。治疗后,两组牙根管壁增厚均厚于治疗前,牙根长度均长于治疗前(P<0.05),但组间比较差异无统计学意义。治疗后,两组龈沟液碱性成纤维细胞生长因子(bFGF)、血管内皮生长因子(VEGF)水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论与TAP相比,Vitapex应用于牙髓血运重建术中的疗效更理想,并对牙根生长发育有促进效果,美观度和患者接受度等均更高,值得临床推广应用。Objective To investigate the clinical efficacy of Vitapex and triple antibiotic paste(TAP)in pulp revascularization.Methods A total of 100 children with periapical periodontitis or young permanent teeth pulpitis admitted to Zaozhuang Stomatological Hospital from April 2019 to October 2020 were selected as the research subjects,they were divided into the control group and the observation group according to random number table method,with 50 cases in each group.The control group was treated with ATP as root canal disinfectant,and the observation group was treated with Vitapex as root canal disinfectant,the clinical efficacy,root growth and development,treatment satisfaction and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group was 96.00%,which was higher than 78.00%in the control group,the difference was statistically significant(P<0.05).The discoloration rate of teeth in the observation group was 2.00%,which was lower than 18.00%in the control group,and the difference was statistically significant(P<0.05).After treatment,the root canal wall thickening of the two groups was thicker than before treatment,and the root length was longer than before treatment(P<0.05),but there was no statistical significance between the groups.After treatment,basic fibroblast growth factor(bFGF),vascular endothelial growth factor(VEGF)levels of the two groups were higher than before treatment,and observation group was higher than control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Compared with TAP,Vitapex is more effective in pulp revascularization,and has promoting effect on root growth and development,it has higher aesthetics and patient acceptance,which is worthy of clinical promotion and application.
