摘 要:目的探讨超声造影征象与乳腺癌分子生物学指标、增殖细胞抗原(Ki-67)及组织学分级、分子分型之间的关系,以预测临床预后。方法选取2020年6月至2023年6月在医院手术或穿刺并进行超声造影检查的女性浸润性乳腺癌患者107例(共107个病灶),观察超声造影征象,收集术后乳腺癌分子生物学指标、Ki-67及组织学分级、分子分型,分析前者与后三者的相关性。结果人表皮生长因子受体2(HER-2)阳性与阴性组织超声造影不均匀增强、增强后边界不清、向心性增强、增强后范围比较,增殖细胞抗原Ki-67阳性与阴性组织超声造影不均匀增强、增强后边界不清、向心性增强、增强后范围比较,不同组织学分级超声造影不均匀增强、增强后边界不清、向心性增强、增强后范围比较,以及不同分子分型超声造影不均匀增强、增强后边界不清、增强后范围比较,差异均有统计学意义(P<0.05)。结论超声造影与乳腺癌分子生物学指标密切相关,对术前开展临床预后评估有提示作用。Objective To investigate the correlation of contrast-enhanced ultrasound signs and molecular biological indicators,proliferating cell antigen(Ki-67),histological grading and molecular typing of breast cancer,so as to predict clinical prognosis.Methods A total of 107 female patients with invasive breast cancer(107 diseases in total)who underwent surgery or puncture and contrast-enhanced ultrasound examination in hospital were selected from June 2020 to June 2023,the contrast-enhanced ultrasound signs were observed,the molecular biological indicators,Ki-67,histological grading and molecular typing of breast cancer were collected after surgery,and the correlations between the molecular biological indicators and Ki-67,histological grading and molecular typing of breast cancer were analyzed.Results The comparison of the contrast-enhanced ultrasound of uneven enhancement,unclear border after enhancement,centripetal enhancement,and enhanced range between human epidermal growth factor receptor 2(HER-2)positive and negative tissues,the comparison of ultrasound contrast of uneven enhancement,unclear border after enhancement,centripetal enhancement,and enhanced range between proliferating cell antigen Ki-67 positive and negative tissues,the comparison of the contrast-enhanced ultrasound of uneven enhancement,unclear border after enhancement,centripetal enhancement,and enhanced range among different histological grades,the comparison of the contrast-enhanced ultrasound of uneven enhancement,unclear border after enhancement,centripetal enhancement,and enhanced range among different molecular subtypes,the differences were statistically significant(P<0.05).Conclusion Contrast-enhanced ultrasound is closely related to the molecular biological indicators of breast cancer,which may be helpful for clinical prognosis evaluation before operation.
