摘　　要：目的 分析研究微小RNA-203(miR-203)、环磷酸腺苷反应元件结合蛋白1(CREB1)在多发性骨髓瘤患者中的表达及与预后的关系。方法 选取2017年6月至2020年6月郑州大学第五附属医院收治的182例多发性骨髓瘤患者作为研究对象,采用荧光定量聚合酶链式反应法检测患者多发性骨髓瘤组织及正常组织的miR-203、CREB1mRNA的表达水平。治疗后对患者进行3年随访,采用Kaplan-Meier生命曲线进行生存分析,并采用Cox回归分析对预后影响因素进行分析。结果 多发性骨髓瘤组织的miR-203表达低于正常组织,CREB1mRNA表达高于正常组织(t=29.412、49.955,均P<0.05);Cox回归分析显示,ISS分期、miR-203表达及CREB1mRNA表达均为多发性骨髓瘤患者无进展生存期与总生存期的危险因素(P<0.05);Kaplan-Meier生命曲线分析结果显示,miR-203高表达患者无进展生存率及总生存率均高于miR-203低表达患者(P<0.05);CREB1mRNA低表达患者无进展生存率及总生存率均高于CREB1mRNA高表达患者(P<0.05)。结论 多发性骨髓瘤患者的miR-203表达水平降低,CREB1表达水平上升,二者与多发性骨髓瘤患者的预后情况相关。Objective To study the expression of microRNA-203(miR-203)and cyclic adenosine monophosphate response element binding protein 1(CREB1)in multiple myeloma patients and their relation-ship with prognosis.Methods A total of 182 patients with multiple myeloma admitted to the Fifth Affiliated Hospital of Zhengzhou University from June 2017 to June 2020 were retrospectively selected as the research sub-jects.The expression levels of miR-203 and CREB1 mRNA in patients with multiple myeloma and normal tis-sues were detected using fluorescence quantitative polymerase chain reaction.The patients were followed up for 3 years after treatment.Kaplan-Meier life curve was used for survival analysis,and cox regression analysis was used to analyze the prognostic factors.Results The expression of miR-203 in multiple myeloma tissues was lower than that in normal tissues,while the expression of CREB1 mRNA was higher than that in normal tissues(t=29.412,49.955,P<0.05).Cox regression analysis showed that ISS stage,miR-203 expression and CREB1 mRNA expression were risk factors for progression-free survival and overall survival of multiple myeloma pa-tients(P<0.05).Kaplan-Meier life curve analysis showed that progression-free survival and overall survival in patients with high miR-203 expression were higher than those with low miR-203 expression(P<0.05).The pro-gression-free survival rate and overall survival rate of patients with low CREB1 mRNA expression were higher than those with high CREB1 mRNA expression(P<0.05).Conclusion The expression level of miR-203 de-creased,while CREB1mRNA increased in patients with multiple myeloma.Both of these factors are related to the prognosis of patients with multiple myeloma.
