摘 要:目的 分析血小板计数水平与老年人群骨密度的相关性。方法 回顾性分析2017至2020年常规体检的老年人1 861例的临床资料,根据是否存在骨质疏松分为骨质疏松组714例和无骨质疏松组1 147例。对比2组血小板计数水平与骨密度值,Pearson相关性分析血小板计数水平与骨密度值相关性;绘制接受者操作特征曲线(ROC)曲线分析血小板计数水平与骨密度值对骨质疏松的预测价值。结果 2组的性别、年龄、白蛋白(ALB)、血糖(GLU)、肌酐(CREA)、尿素氮(UREA)、糖化血红蛋白(HbA1c)、血清总蛋白(TP)、红细胞(RBC)、血红蛋白(HGB)差异无统计学意义(P>0.05);Pearson相关性分析显示,血小板计数与骨密度呈负相关(r=-0.726,P<0.05);绘制ROC曲线,显示,血小板计数与骨密度及联合预测骨质疏松发生的AUC分别为0.777(0.757~0.798)、0.828(0.807~0.849)、0.835(0.817~0.852),具有一定预测价值。结论 血小板计数水平与骨密度呈负相关,二者对骨质疏松均有一定的预测价值,但通过联合诊断的效能更高。Objective To analyze the correlation between platelet count(PLT)and bone mineral density(BMD)in the elderly.Methods Clinical data of 1,861 elderly patients who underwent routine physical examination in our hospital from January 2017 to December 2020 were retrospectively analyzed.They were assigned into osteoporosis group(n=714)and non-os teoporosis group(n=1,147).PLT and BMD were compared between groups,and their correlation was assessed by Pearson correlation.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of PLT and BMD in osteoporosis.Results There were no significant differences in gender,age,albumin(ALB),blood glucose(GLU),creatinine(CREA),urea nitrogen,glycated hemoglobin(HbA1c),total serum protein(TP),red blood cell count(RBC)and hemoglobin(HGB)between osteoporosis group and non-osteoporosis group(P>0.05).Pearson correlation analysis showed that PLT had a negative correlation with BMD(r<0.05,P<0.05).The ROC curve was drawn,and the results showed that the area under the curve(AUC)of PLT,BMD and their combination in discriminating osteoporosis was 0.777(0.757~0.798),0.828(0.807~0.849)and 0.835(0.817~0.852),respectively,presenting a certain predictive value.Conclusion Platelet count has a negative correlation with bone mineral density,both of which have certain predictive value for osteoporosis.Their combination detection is more effective in the diagnosis of osteoporosis.
