摘 要:目的研究分析有晶状体眼后房型人工晶状体(ICL)植入术后早期的拱高与术前眼前节参数的相关性。方法回顾性病例研究。收集2021年7月至2022年10月行ICL植入手术患者388例(696只眼)的病历资料,所有患者术后随访1周,使用三维眼前节分析仪(Pentacam)测量患者术后拱高,分析ICL术后早期拱高与术前角膜水平直径即白到白(WTW)、平均睫状沟直径(STS)、房角距离(ATA)、前房深度(ACD)、术前等效球镜(SE)、眼轴长度(AL)、晶状体厚度(LT)、晶状体矢高(CLR)以及晶状体前表面顶点到水平睫状沟直径所在平面的距离(STSL)之间的相关性,并通过逐步多元线性回归分析建立预测拱高的线性回归方程。结果患者行ICL植入术后1周裸眼视力为-0.08(-0.08,<0.00)logMAR,较术前1.00(1.00,1.22)logMAR显著提升,差异有统计学意义(Z=-22.892,P<0.001),术后1周的拱高与WTW(r=0.229,P<0.001)、ACD(r=0.296,P<0.001)、STS(r=0.163,P<0.001)、AL(r=0.079,P<0.05)、ICL尺寸(r=0.210,P<0.001)呈正相关,与STSL(r=-0.163,P<0.001)、CLR(r=-0.203,P<0.001)、LT(r=-0.292,P<0.001)呈负相关,与SE和ATA无显著相关性(P>0.05)。线性回归结果显示:影响和预测ICL术后早期拱高的因素有ACD、STSL、LT、ICL尺寸。线性回归方程:拱高=-209.742+116.764×(ACD)-114.234×(STSL)-106.168×(LT)+58.156×ICL尺寸(R^(2)=0.130)。结论术后1周的拱高与WTW、ACD、STS、AL、ICL尺寸呈正相关,与STSL、CLR、LT呈负相关,与SE和ATA不相关,ACD、STSL、LT和ICL尺寸是影响和预测术后拱高的重要因素。Objective To analyse the correlation between early postoperative vault and preoperative anterior seg⁃ment parameters in myopic patients who underwent implantable collamer lens(ICL)implantation in lensed eyes.Methods A retrospective case study was undertaken to collect medical records of 696 eyes of 388 patients who underwent ICL im⁃plantation between July 2021 and October 2022.And postoperative follow-up extended to one week.The vault was mea⁃sured using Pentacam.The study elucidated the correlation between the vault during the early postoperative phase and the preoperative white-to-white distance(WTW),mean sulcus-to-sulcus distance(STS),iridocorneal angle to iridocorneal angle measurement(ATA),anterior chamber depth(ACD),spherical equivalent(SE),axial length(AL),lens thickness(LT),crystalline lens rise(CLR),and the distance between the STS plant and the anterior crystalline lens surface(STSL).The derivation of a predictive linear regression equation for vault was achieved through stepwise multiple linear regression analysis.Results The uncorrected visual acuity at 1week post-ICL implantation demonstrated a marked improvement from the preoperative level of 1.00(1.00,1.22)logMAR to the postoperative of-0.08(-0.08,0)logMAR(Z=-22.892,P<0.001).The vault at 1week postoperatively exhibited positive correlation with WTW(r=0.229,P<0.001),ACD(r=0.296,P<0.001),STS(r=0.163,P<0.001),AL(r=0.079,P<0.05),ICLdimensions(r=0.210,P<0.001),while manifesting negatively correlated with STSL(r=-0.163,P<0.001),CLR(r=-0.203,P<0.001),and LT(r=-0.292,P<0.001).No sig⁃nificant correlations were discerned with preoperative SE or ATA(P>0.05).Linear regression analysis revealed that the de⁃terminants influencing early vault post-ICL implantation were ACD,STSL,LT,and ICL size.The derived linear regression equation is expressed as follows:vault=-209.742+116.764×(ACD)-114.234×(STSL)-106.168×(LT)+58.156×ICL dimen⁃sions(R^(2)=0.130).Conclusions The vault at 1week post-ICL implantation positively correlated with WTW,ACD,STS,AL,and ICL
