摘  要:目的探讨肿瘤坏死因子-α(TNF-α)、非编码单链RNA-19a(miR-19a)、单次跨膜糖蛋白147(CD147)在急性脑梗死(ACI)伴脑微出血(CMB)患者中的诊断价值。方法选取2020年1月至2022年1月广元市中心医院神经内科收治的140例ACI患者,根据是否合并CMB分组,比较有无CMB患者血清中的TNF-α、miR-19a、CD147的表达水平,并分析上述这些指标表达水平在ACI合并CMB中的诊断价值。结果CMB组血清中的TNF-α、miR-19a、CD147相比无CMB组患者都有明显升高(P<0.05);Spearman等级相关分析CMB严重程度与TNF-α、miR-19a、CD147表达水平呈正相关(r=0.67~0.89,P<0.05);以SIEMENS MAGNETOM 3.0T超导MRI扫描仪和Kim评定标准为金标准,判断外周血TNF-α、miR-19a、CD147对CMB的诊断价值。受试者工作特征曲线(ROC)分析结果显示,单指标用于CMB诊断价值曲线下面积分别为0.725,0.753、0.748,联合诊断时曲线下面积为0.865。结论对ACI患者血清TNF-α、miR-19a、CD147水平进行联合检测,能够增加检测效能,为及早实现CMB治疗个性化方案的制定提供可靠的线索。Objective To investigate the necrosis factor-α(TNF-α),MicroRNA-19a(miR-19a),and single transmemhrane glycoprotein 147(CD147)in acute cerebral infarction(ACI),the diagnostic value to infarct,ACIin patients with microbleeding.Methods A total of 140 ACI patients admitted to Department of Neurology,Guangyuan Central Hospital from January 2020 to January 2022 were selected and grouped according to whether they were combined with microhleeding.The expression levels of TNF-α,miR-19a and CD 147 in the serum of patients with microbleeding were compared,and the diagnostic value of these expression levels in ACI combined with microbleeding was analyzed.Results The serum levels of TNF-α,miR-19a and CD 147 in CMB group were significantly higher than those in no CMB group,and the difference was statistically significant(P<0.05).Spearman correlation analysis showed that the severity of CMB was positively corr-elated with the expression levels of TNF-α,miR-19a and CD147(r=0.67-0.89,P<0.05).The diagnostic value of TNF-α,miR-19a and CD147 in peripheral blood for CMB was determined by SIEMENS MAGNETOM 3.0T superconducting MRI scanner and Kim evaluation standard.ROC analysis results showed that the area under the curve of CMB diagnostic value for single index was 0.725,0.753,0.748,respectively,and the area under the curve for combined diagnosis was 0.865.Conclusion The combined detection of serum TNF-α,miR-19a and CD 147 levels in patients with cerebral infarction can increase the detection efficiency and provide reliable clues for the early realization of individualized CMB treatment.
