摘　　要：目的:分析神经节苷脂联合苁蓉益智胶囊治疗脑梗死后血管性痴呆(VD)的效果。方法:选取2021年1月—2023年1月北京市顺义区医院收治的脑梗死后VD患者104例作为研究对象,采用随机数字表法分为对照组与研究组,各52例。对照组给予神经节苷脂治疗,研究组在对照组基础上给予复方苁蓉益智胶囊治疗。比较两组治疗效果。结果:治疗后,两组血细胞比容、血浆黏度、全血低切黏度、全血高切黏度低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组大脑前动脉、大脑后动脉、大脑中动脉、椎动脉平均血流速度大于治疗前,且研究组大于对照组,差异有统计学意义(P<0.05)。治疗后,两组蒙特利尔认知评估量表、日常活动能力量表评分高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:神经节苷脂联合苁蓉益智胶囊治疗脑梗死后VD的效果显著,能够改善血液流变学、血流动力学指标,改善患者认知功能,提高生活能力,值得临床应用并予以推广。Objective:To analyze the effect of ganglioside combined with Congrong Yizhi Capsules in the treatment of vascular dementia(VD)after cerebral infarction.Methods:A total of 104 patients with VD after cerebral infarction admitted to Beijing Shunyi Hospital from January 2021 to January 2023 were selected as the study subjects.They were divided into control group and study group according to random number table method,with 52 cases in each group.The control group was treated with ganglioside,and the study group was treated with Compound Congrong Yizhi Capsules on the basis of the control group.The treatment effects were compared between the two groups.Results:After treatment,the hematocrit,plasma viscosity,whole blood low-shear viscosity and whole blood high-shear viscosity in the two groups were lower than those before treatment,these indexes in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the average blood flow velocity of the anterior cerebral artery,posterior cerebral artery,middle cerebral artery and vertebral artery in the two groups was higher than that before treatment,this index in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of Montreal Cognitive Assessment Scale and Activities of Daily Living Scale in the two groups were higher than those before treatment,the scores in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Ganglioside combined with Congrong Yizhi Capsules have a significant effect in the treatment of VD after cerebral infarction,and can ameliorate hemorheological and hemodynamic indexes,improve patients'cognitive function,enhance living ability,which is worthy of clinical application and promotion.
