摘　　要：目的:分析改良自取保存羊膜手术对复发性翼状胬肉患者的影响。方法:回顾性选取2021年1月—2023年6月福建医科大学附属三明第一医院收治的68例(68眼)复发性翼状胬肉患者。根据不同手术方式将其分为A组(32例,32眼)和B组(36例,36眼)。A组给予常规羊膜移植手术,B组给予改良自取保存羊膜手术。比较两组相关指标,术前和术后1周、1个月、2个月干眼情况,手术满意度。结果:术后第1天,B组角膜刺激症状发生率低于A组,角膜上皮完全愈合时间短于A组,差异有统计学意义(P<0.05)。两组术前非接触泪膜破裂时间(non contact tear film break-up time,nBUT)、泪液分泌试验(SchirmerⅠtest,SⅠt)比较,差异无统计学意义(P>0.05);术后1周、1个月,B组nBUT、SⅠt均高于A组,差异有统计学意义(P<0.05)。两组nBUT、SⅠt组间、时间、时间×组间比较,差异有统计学意义(P<0.05)。B组手术满意度高于A组,差异有统计学意义(P<0.05)。结论:改良自取保存羊膜手术能明显改善复发性翼状胬肉患者术后的角膜刺激症状,促进角膜损伤的修复,改善患者干眼,减轻患者术后的不适,提高患者的满意度,但术后远期干眼改善有待进一步观察。Objective:To analyze the effect of modified self pickup amniotic membrane preservation operation on patients with recurrent pterygium.Method:Sixty-eight patients(68 eyes)with recurrent pterygium treated in the Affiliated Sanming First Hospital of Fujian Medical University from January 2021 to June 2023 were retrospectively selected.According to different surgical methods,they were divided into group A(32 cases,32 eyes)and group B(36 cases,36 eyes).Group A received conventional amniotic membrane transplantation operation and group B received modified self pickup amniotic membrane preservation operation.The relevant indexes,the dry eye condition before surgery and 1 week,1 month and 2 months after operation,the satisfaction of operation of the two groups were compared.Result:On the first day after operation,the incidence of corneal irritation symptoms in group B was lower than that in group A,and the complete healing time of corneal epithelium was shorter than that in group A,the differences were statistically significant(P<0.05).There were no significant differences in preoperative non contact tear film break-up time(nBUT),SchirmerⅠtest(SⅠt)between the two groups(P>0.05).At 1 week and 1 month after operation,nBUT and SⅠt in group B were higher than those in group A,and the differences were statistically significant(P<0.05).There were statistically significant differences in nBUT,SⅠt,time,intergroup,time×intergroup between the two groups(P<0.05).The surgery satisfaction of group B was higher than that of group A,the difference was statistically significant(P<0.05).Conclusion:Modified self-preservation amniotic membrane operation can significantly improve postoperative corneal irritation symptoms in patients with recurrent pterygium,promote the repair of corneal injury,improve patients'dry eye,reduce postoperative discomfort,and increase patient satisfaction.However,long-term postoperative dry eye improvement remains to be further observed.
