摘 要:外周T细胞淋巴瘤(PTCL)是一组具有高度异质性的血液系统恶性肿瘤。目前一线治疗通常采用CHOP(环磷酰胺、长春新碱、泼尼松、阿霉素)或CHOP样方案,但缓解率低,预后差,亟待在传统的化疗方案基础上改良以提高疗效。近年来研究发现,PTCL发病机制中表观遗传学的改变至关重要,越来越多的新型表观遗传学靶向药物在PTCL的单药及联合治疗中展现出良好的效果和安全性,为PTCL的精准诊治提供了新的策略。该文就PTCL表观遗传学改变最新进展及靶向药物治疗应用前景进行综述,以期为改善PTCL患者的临床预后提供参考。Peripheral T-cell lymphoma(PTCL) is a group of hematological malignant tumors with high heterogeneity.At present,CHOP(cyclophosphamide,vincristine,prednisone,doxorubicin) or CHOP-like regimens are usually used for first-line treatment,but the remission rate is low,and the prognosis is poor.It is urgent to improve the traditional chemotherapy regimens to improve the efficacy onthe basis of tradtional chemotherapeutic regimen.Recent studies have found that the epigenetic alterations are crucial in the pathogenesis of PTCL,and more and more new epigenetic targeted drugs have shown good effects and safety in the monotherapy and combination therapy of PTCL,which provide a new strategy for the precise diagnosis and treatment of PTCL.This article reviews the latest progress in epigenetic changes of PTCL and the prospects of targeted drug therapy applications,in order to provide reference for improving the clinical prognosis of the patients with PTCL.
