摘　　要：目的探讨三阴性乳腺癌患者雄激素受体、锌指蛋白44、p53蛋白的表达特点及意义。方法选取三阴性乳腺癌患者60例(三阴性乳腺癌组)、非三阴性乳腺癌患者74例(非三阴性乳腺癌组),两组患者行乳腺穿刺病理活检,利用免疫组化法检测雄激素受体、锌指蛋白44、p53蛋白表达;收集三阴性乳腺癌患者组织学分级、肿瘤直径、脉管内浸润、淋巴结转移情况。进行1 a随访,根据随访结果将患者分为预后良好组和预后不良组。结果三阴性乳腺癌组患者雄激素受体阳性表达率、锌指蛋白44阳性表达率明显低于非三阴性乳腺癌组,p53蛋白阳性表达率明显高于非三阴性乳腺癌组(均P<0.01)。组织学分级Ⅱ级、有淋巴结转移的三阴性乳腺癌患者雄激素受体、锌指蛋白44表达阳性率明显低于织学分级Ⅰ级、无淋巴结转移的患者,p53蛋白表达阳性率明显高于织学分级Ⅰ级、无淋巴结转移的患者(均P<0.05)。雄激素受体、锌指蛋白44、p53蛋白表达阳性率在不同年龄、肿瘤直径、脉管内浸润的三阴性乳腺癌患者间比较差异无统计学意义(P>0.05)。三阴性乳腺癌患者中预后不良组雄激素受体、锌指蛋白44表达阳性率明显低于预后良好组,p53蛋白表达阳性率明显高于预后良好组(均P<0.01)。预后不良组三阴性乳腺癌患者组织学分级Ⅱ级比例、有脉管内浸润、有淋巴结转移明显高于组织学分级Ⅰ级比例、无脉管内浸润、无淋巴结转移的患者(P<0.01)。年龄、肿瘤直径在不同预后的三阴性乳腺癌患者之间比较差异无统计学意义(P>0.05)。激素受体、锌指蛋白44、p53蛋白、组织学分级、脉管内浸润、淋巴结转移是三阴性乳腺癌患者预后的独立影响因素。ROC曲线分析显示,雄激素受体、锌指蛋白44、p53蛋白联合检测的预测价值最高(AUC=0.960),其次为p53蛋白、雄激素受体、锌指蛋白44单独检测。结论三阴性�Objective To investigate the expression characteristics and significance of androgen receptor,zinc finger protein 44 and p53 protein in triple negative breast cancer patients.Method 60 triple negative breast cancer patients and 74 non triple negative breast cancer patients were selected.Perform breast biopsy and immunohistochemistry to detect the expression of androgen receptor,zinc finger protein 44 and p53 protein;The histological grading,tumor diameter,vascular invasion and lymph node metastasis of triple negative breast cancer patients were collected.Conduct a one-year follow-up,and divide into a good prognosis group and a poor prognosis group.Results The positive expression rate of androgen receptor and zinc finger protein 44 in the triple negative breast cancer group were significantly lower than those in the non triple negative breast cancer group,and the positive expression rate of p53 protein was significantly higher than that in the non triple negative breast cancer group(all P<0.01).The positive rate of androgen receptor and zinc finger protein 44 expression in triple negative breast cancer patients with histological gradeⅡand lymph node metastasis was significantly lower than that in patients with histological gradeⅠand no lymph node metastasis,and the positive rate of p53 protein expression was significantly higher than that in patients with histological grade I and no lymph node metastasis(all P<0.05).The positive rates of androgen receptor,zinc finger protein 44 and p53 protein expression were not significantly different among three negative breast cancer patients of different ages,tumor diameters and vascular invasion(P>0.05).The positive rate of androgen receptor and zinc finger protein 44 expression in poor prognosis group was significantly lower than that in good prognosis group,and the positive rate of p53 protein expression was significantly higher than that in good prognosis group(all P<0.01).There was no significant difference in age and tumor diameter between three negative breast canc
