320排螺旋CT对胃癌淋巴结转移的诊断价值分析

摘　　要：目的探讨320排螺旋CT对胃癌患者淋巴结转移的诊断价值。方法选取2020年11月至2021年11月麻城市人民医院收治的100例胃癌患者作为研究对象,患者均行320排螺旋CT扫描观察患者胃周淋巴结转移情况。以术后病理结果为金标准,分析320排螺旋CT扫描诊断淋巴结转移的诊断效能,分析胃癌淋巴结转移影像学特征,比较淋巴结转移与非淋巴结转移CT参数,ROC分析320排螺旋CT影对胃癌淋巴结转移的诊断效能。结果区域内淋巴结肿大,通常呈环状、囊状或多个淋巴结融合形成的实质块,环状和囊状多见于较大淋巴结,整体上以环状多见,增强扫描呈环状强化,中心相对低密度,周边高密度,动脉期明显,静脉期下降;囊状病灶呈中心低密度周边高密度,轻中度不均匀明显强化;实质块由淋巴结融合形成,呈较均匀的轻中度强化。100例患者中,320排螺旋CT检查N0期、N1期、N2期、N3期Kappa值分别为0.763、0.724、0.787、0.735,与术后病理诊断一致性较好。淋巴结转移患者短径长于非淋巴结转移患者,短径/长径大于非淋巴结转移患者,强化程度均高于非淋巴结转移患者,差异有统计学意义(P<0.05)。ROC曲线分析结果显示,320排螺旋CT参数联合诊断明显优于单一诊断。结论320排螺旋CT能清晰观察胃癌淋巴结转移病灶的形态、大小、强化程度及密度变化,对患者N分期有较高的评估价值,与术后病理一致性较好,对胃癌患者淋巴结转移有较高的诊断价值。Objective To investigate the Diagnostic value of 320-slice spiral CT in lymph node metastasis of gastric cancer.Methods 100 pa-tients with gastric cancer admitted to Macheng People's Hospital from November 2020 to November 2021 were selected as the research subjects,all patients underwent 320-slice spiral CT scan to observe the metastasis of perigastric lymph nodes.Taking the postoperative pathological results as the gold standard,the diagnostic efficacy of 320-slice spiral CT scan in the diagnosis of lymph node metastasis was analyzed,the imaging features of lymph node metastasis in gastric cancer were analyzed,the CT parameters of lymph node metastasis and non-lymph node metastasis were compared,the diagnostic efficacy of 320-slice spiral CT shadow in lymph node metastasis of gastric cancer was analyzed by ROC.Results The lymph nodes in the region were enlarged,usually in a ring,cystic or solid mass formed by the fusion of multiple lymph nodes.The ring and cystic were more com-mon in the larger lymph nodes,and the ring was more common as a whole,the enhanced scan showed ring enhancement,the center was relatively low density,the periphery was high density,the arterial phase was obvious,and the venous phase was decreased;cystic lesions showed low density in the center and high density in the periphery,with mild to moderate uneven enhancement;the solid mass was formed by lymph node fusion and showed relatively uniform mild to moderate enhancement.Among the 100 patients,the Kappa values of 320-slice spiral CT in N0,N1,N2 and N3 were 0.763,0.724,0.787 and 0.735,respectively,which were in good agreement with postoperative pathological diagnosis.The short diameter of pa-tients with lymph node metastasis was longer than that of patients without lymph node metastasis,the short diameter/long diameter was larger than that of patients without lymph node metastasis,and the degree of enhancement was higher than that of patients without lymph node metastasis,the differences were statistically significant(P<0.05).ROC curve ana
