经腹部超声联合经阴道超声在产前诊断血管前置中的检出率及临床价值分析

摘　　要：目的探讨经腹部超声联合经阴道超声联合诊断血管前置的检出率及临床价值。方法回顾性分析2019年5月至2022年5月枣庄市中医医院收治的100例疑诊血管前置行剖宫产术患者的临床资料。所有患者均先后予以经腹部超声、经阴道超声检查,以术后病理结果为诊断金标准,分析经腹部超声、联合超声检查对血管前置的诊断结果及诊断效能。结果100例患者剖宫产术后病理结果显示,血管前置85例,占比85.00%,无血管前置15例,占比15.00%;经腹部超声检查,血管前置检出87例,检出率为87.00%,其中误诊6例,漏诊4例;联合超声诊断,血管前置检出86例,检出率为86.00%,其中误诊4例,漏诊3例;以术后病理结果为金标准,经腹部超声诊断的灵敏度为95.29%,特异度为60.00%,准确度为90.00%;经腹部超声联合经阴道超声诊断的灵敏度为96.47%,特异度为93.33%,准确度为93.00%。联合超声诊断的特异度高于经腹部超声(P<0.05),两种检查方法的诊断灵敏度、准确度比较差异无统计学意义。结论经腹部超声联合经阴道超声在血管前置产前诊断中具有显著价值,可降低干扰因素,提高诊断灵敏度、准确度,从而为疾病诊断、手术实施提供影像学辅助及依据。Objective To investigate the clinical value and detection rate of transabdominal ultrasound combined with transvaginal ultrasound in prenatal diagnosis of vascular previa.Methods The clinical data of 100 patients with suspected vascular previa undergoing cesarean section admit-ted to Zaozhuang Hospital of Traditional Chinese Medicine from May 2019 to May 2022 were retrospectively analyzed.All patients were examined by transabdominal ultrasound and transvaginal ultrasound successively,taking the postoperative pathological results as the diagnostic gold standard,the diagnostic results and diagnostic efficacy of transabdominal ultrasound and combined ultrasound for vasa previa were analyzed.Results After cesarean section of 100 patients,86 patients had vascular previa,accounting for 86.00%,and 15 patients had no vascular previa,accounting for 15.00%;transabdominal ultrasonography detected vascular previa in 87 cases,and The detection rate was 87.00%,of which 6 cases were misdiag-nosed and 4 cases were misdiagnosed;combined with ultrasound diagnosis,85 cases were detected with vascular anterior,and the detection rate was 85.00%,of which 4 cases were misdiagnosed and 3 cases were missed.Using postoperative pathological results as the gold standard,the sensitivity of transabdominal ultrasound diagnosis was 95.29%,the specificity was 60.00%,and the accuracy was 90.00%.The sensitivity of transabdominal ul-trasound combined with transvaginal ultrasound was 96.47%,the specificity was 93.33%,and the accuracy was 93.00%.The specificity of combined ultrasound diagnosis was higher than that of transabdominal ultrasound(P<0.05),there was no significant difference in diagnostic sensitivity and ac-curacy between the two methods.Conclusion Transabdominal ultrasound combined with transvaginal ultrasound has significant value in prenatal di-agnosis of vascular previa,which can reduce interference factors and improve diagnostic sensitivity and accuracy,thereby providing imaging assis-tance and basis for disease diagnosis and surgical
