大活络胶囊配合循经针灸对中风后痉挛性偏瘫患者肌电生理指标的影响

摘　　要：目的 探究大活络胶囊配合循经针灸对中风后痉挛性偏瘫患者肌电生理指标的影响。方法 选取2021年6月至2022年6月萍乡矿业集团有限责任公司总医院收治的80例中风后痉挛性偏瘫患者,按随机数字表法分为两组,各40例。对照组采取中风基础治疗及康复训练,在此基础上,观察组采取大活络胶囊配合循经针灸。比较两组治疗前、治疗8周的肌电生理指标[均方根值(RMS)、表面肌电值(iEMG)、中位频率(MF)、H反射H/M值],肢体功能[Fugl-Meyer运动功能评定(FMA)],日常生活活动能力[改良Barthel指数(BI)]。结果 治疗后,两组iEMG、MF、RMS高于治疗前,观察组比对照组高,H反射H/M值低于治疗前,观察组比对照组低(P<0.05);两组FMA量表总分、改良BI指数比治疗前高,观察组比对照组高(P<0.05)。结论 大活络胶囊配合循经针灸可改善中风后痉挛性偏瘫患者肌电生理指标及肢体功能,提高患者的日常生活活动能力。Objective To explore the effect of dahuoluo capsules combined with acupuncture on electromyophysiological indexes in patients with spastic hemiplegia after stroke.Methods 80 patients with spastic hemiplegia after stroke treated in Pingxiang Mining Group Co.,LTD.General Hospital from June 2021 to June 2022 were selected and divided into two groups according to the random number table method,with 40 cases in each group.The control group received basic treatment and rehabilitation training for stroke.On this basis,the observation group received dahuoluo capsules combined with acupuncture and moxibustion.Myoelectrophysiological indexes[root mean square value(RMS),surface electromyography value(iEMG),median frequency(MF),H-reflex H/M value],limb function[Fugl-Meyer Motor function Assessment(FMA)],and activity of daily living[modified Barthel index(BI)]of the two groups were compared before treatment and at 8 weeks.Results After 8 weeks of treatment,iEMG,MF and RMS in both groups were higher than before treatment,the observation group was higher than control group,the H-reflex H/M value was lower than before treatment,and the observation group was lower than control group(P<0.05);the total score of FMA scale and improved BI index in both groups were higher than before treatment,and the observation group was higher than the control group(P 0.05).Conclusion Dahuoluo capsules combined with acupuncture and moxibustion can improve the electromyophysiological indexes and limb function of patients with spastic hemiplegia after stroke,and improve the ability of daily living.
