宫颈癌筛查中TCT联合高危型HPV检测的作用

摘　　要：目的探讨宫颈癌筛查中液基薄层细胞检测(TCT)+高危型人乳头瘤病毒(HPV)检测的作用。方法选取120例于本院进行宫颈癌筛查的女性为研究对象,均开展TCT联合高危型HPV检测。对筛查结果进行统计,并分析检出率及诊断效能。结果以病理检查结果为金标准。120例受检者中,高危型HPV阳性者79例,与病理检测结果阳性符合率为92.41%,阴性符合率为68.29%;TCT阳性者74例,与病理检测结果阳性符合率为89.19%,阴性符合率为76.09%;联合检测对于宫颈上皮内瘤变(CIN)Ⅰ、CINⅡ、CINⅢ、鳞状细胞癌的检出率均高于单一检测(P<0.05)。TCT检测的敏感性、特异性、准确性分别为89.61%、88.37%、89.17%;高危型HPV检测的敏感性、特异性、准确性分别为93.51%、83.72%、90.00%;联合检测的敏感性、特异性、准确性分别为94.81%、93.02%、94.17%;联合检测的敏感性、特异性、准确性均略高于单独检测,但差异无统计学意义(P>0.05)。结论宫颈癌筛查中TCT联合高危型HPV检测的应用价值较高,能提高宫颈癌和癌前病变的检出率,降低误诊、漏诊发生率。Objective To investigate the role of thinprep cytologic test(TCT)+with high-risk human papillomavirus(HPV)in cervical cancer screening.Methods 120 women who underwent cervical cancer screening in the hospital were selected as the study objects,and all of them underwent TCT combined with high-risk HPV detection.The screening results were statistically analyzed,and the detection rate and diagnostic efficiency were analyzed.Results The results of pathological examination were the gold standard.Among 120 cases,79 cases were high risk HPV positive,the positive coincidence rate with pathological examination was 92.41% and the negative coincidence rate was 68.29%;74 cases were TCT positive,the positive coincidence rate was 89.19%and the negative coincidence rate was 76.09%;the detection rates of cervical intraepithelial neoplasia(CIN)I CIN II,CIN II and squamous cell carcinoma by combined detection were higher than those by single detections(P<0.05).The sensitivity,specificity and accuracy of TcT were 89.61%,88.37% and 89.17%,respectively;and were 93.51%,83.72% and 90.00%of high-risk HPV detection;and were 94.81%,93.02% and 94.17%of combined detection;the sensitivity,specificity and accuracy of combined detection were slightly higher than those of single detections,but the differences were not statistically significant(P>0.05).Conclusion The application value of TCT combined with high-risk HPV detection in cervical cancer screening is high,which can improve the detection rate of cervical cancer and precancerous lesions,and reduce the incidence of misdiagnosis and missed diagnosis.
