摘  要:目的探討嗜酸性粒細胞在結直腸息肉和腫瘤組織中的分佈及意義。方法選擇2022年6月~2023年6月鏡湖醫院腸鏡檢查並行結直腸息肉摘除的患者採用光學顯微鏡觀察結直腸息肉組織中嗜酸性粒細胞的分佈和數量。結果共860例,包括結直腸腺癌52例,高級别異型增生腺瘤78例,低級别異型增生腺瘤481例,增生性息肉249例。嗜酸性粒細胞彌漫分佈於息肉的間質組織。在249例非腫瘤組(增生性息肉)中,>20/HPF嗜酸性粒細胞的病例為81例(32.5%);在611例腫瘤組(包括腺癌,高級别和低級别異型增生腺瘤)中,>20/HPF嗜酸性粒細胞的病例為600例(98.2%)。非腫瘤組與腫瘤組嗜酸性粒細胞的分佈和數量差異在統計學上有顯著性意義(P<0.01)。在結直腸腺癌、高級别異型增生腺瘤、低級别異型增生腺瘤中,>20/HPF嗜酸性粒細胞的病例分别佔96.2%(50/52),98.7%(77/78),98.3%(473/481)。各級别腫瘤組之間嗜酸性粒細胞數量的差異,在統計學上無顯著性意義(P>0.05)。結論嗜酸性粒細胞的分佈和數量與結直腸腫瘤形成有明顯的相關性。Objective To investigate the significance of eosinophils in colorectal polyp and tumor tissues.Methods The distribution and number of eosinophils in colorectal polyps were observed by light microscope.Results A total of 860 patients underwent colonoscopy and colorectal polyp removal in KiangWu Hospital from June 2022 to June 2023 were collected,including 52 cases of colorectal carcinoma,78 cases of high-grade dysplastic adenoma,481cases of low-grade dysplastic adenoma,and 249 cases of hyperplastic polyp.Eosinophils were diffusely infiltratein the stroma of the polyps.81 cases of>20/HPF eosinophils were found in 249 cases(32.5%)of non-tumor group(hyperplastic polyp),while 600/611(98.2%)cases in tumor group(including colorectal carcinoma,high-grade and low-grade dysplastic adenoma).The difference of eosinophil number between non-tumor group and tumor group was statistically significant(P<0.01).>20/HPF eosinophils were found in 96.2%(50/52),98.7%(77/78)and 98.3%(473/481)of colorectal cancer,high-grade dysplastic adenoma,and low-grade dysplastic adenomarespectively.The difference of eosinophil number betweendifferent grade tumor groups wasnot statistically significant(P>0.05).Conclusion The presence of eosinophils was significantly correlated with formation of colorectal tumors.The mechanism of promoting tumor growth needs further explore.
